摘要
目的:梳理武之望《济阴纲目》对诊治带下病六类证候(风邪、湿热、湿痰、虚损、虚寒、滑脱)的用药规律,以数据佐证说明武氏治疗带下病的用药心得。方法:按武之望提出的带下病六类证候统计用药规律频次及计算频率,比较带下病综合用药规律及各类证候用药规律频率的异同,结合原文理论分析及论述各类证候用药的特色,并举例说明当代医家如何传承其学术及用药特点治疗带下病。结果:武之望《济阴纲目》带下门处方共63首,用药133味,总用药频次494次。各证候带下病药物药类、性味、归经频率有明显不同侧重点:(1)虚寒带下多温经补血敛涩;(2)虚损带下多健脾升阳除湿;(3)湿热带下多苦寒辛温并用;(4)带下滑脱多固涩止带,少用峻烈之品;(5)湿痰带下分标本虚实,重用苦辛药物;(6)风邪带下多用辛温,随证治之。结论:武之望重视妇人以血为本、以气为用,治疗带下病强调因证立方,重视辨治虚实,尤以湿热、湿痰带下用药为特色,为中医治疗带下病提供了临床依据。
Objective:To study and analyze the syndrome diagnosis and differential treatment of leukorrhea in the ancient gynecological literature Outline for Women's Diseases,written by Wu Zhiwang in Ming dynasty.Methods:The database was established with Microsoft Office Excel 365 software.Six types of syndromes(wind-evil,dampness-heat,dampness-phlegm,asthenia(weakness),deficiency of Yang with cold,and spontaneous vaginal discharge)were identified.The similarities and differences in the prescriptions for different syndromes of leukorrhea were compared.Results:The ancient gynecological literature Outline for Women's Diseases recorded 63 prescriptions for treating leukorrhea,with 133 medicinal herbs or substances.The medicine mainly consisted of the warm properties with bitter,pungent,sweet tastes characteristics,and tropisms to the spleen,liver,kidney,and stomach meridians.The five highest frequency Chinese medicine recorded are Angelica sinensis,Radix Paeoniae Rubra,Licorice,Chuanxiong,and Phellodendron amurense.The treatment of leukorrhea associated with different syndromes have different strategies and prescription characteristics.Conclusion:Wu emphasized the importance of blood and Qi in women.Different syndromes have their own treatment features.The combination analysis and interpretation of both medicinal descriptive data and theory of Chinese medicine provided a better picture about Wu's experience and ideas in the treatment of leukorrhea.The study is expected to provide clinical thinking and basis for the treatment of ailments in Chinese medicine.
