摘要
目的观察注射用醋酸亮丙瑞林微球辅助治疗青春期特发性矮小症(ISS)的效果。方法选取2018年6月—2021年3月晋江市妇幼保健院和石狮市妇幼保健院收治的青春期ISS患儿60例,按照随机数字表法分为试验组和对照组,每组30例。对照组给予注射用重组人生长激素治疗,试验组在对照组基础上予注射用醋酸亮丙瑞林微球治疗,2组均连续治疗12个月。比较2组患儿治疗前后生长速率、预测终身高、骨龄、糖化血红蛋白、空腹血糖、甲状腺功能与血清胰岛素样生长因子结合蛋白-3(IGFBP-3)、胰岛素样生长因子-1(IGF-1)水平及并发症。结果治疗12个月后,2组患儿生长速率、预测终身高较治疗前增加,且试验组生长速率、预测终身高高于对照组(P均<0.01);2组患儿骨龄较治疗前有所增加,但试验组骨龄低于对照组(P均<0.01)。试验组患儿骨龄增长(0.48±0.17)年,慢于对照组的(0.96±0.14)年(t=11.938,P<0.001)。2组患儿治疗前后糖化血红蛋白、空腹血糖、促甲状腺素、总甲状腺素、总三碘甲腺原氨酸水平比较差异均无统计学意义(P>0.05)。治疗12个月后,2组患儿血清IGFBP-3、IGF-1水平较治疗前升高(P均<0.01),但2组患儿治疗后水平比较差异无统计学意义(P>0.05)。试验组与对照组患儿并发症总发生率比较差异无统计学意义(6.67%vs.13.33%,χ^(2)=0.185,P=0.667)。结论注射用醋酸亮丙瑞林微球辅助治疗青春期ISS的效果明显,有利于患儿生长发育,增加预测终身高,且对血糖及甲状腺功能无不良影响,可调节血清IGFBP-3及IGF-1水平,具有一定安全性。
Objective To observe the effect of leuprorelin acetate microspheres for injection in the adjuvant treatment of adolescent idiopathic short stature(ISS).Methods Sixty adolescent ISS children admitted to Jinjiang Maternal and Child Health Hospital and Shishi Maternal and Child Health Hospital from June 2018 to March 2021 were selected and randomly divided into experimental group and control group by random number table method,with 30 cases in each group.The control group was given recombinant human growth hormone,while the experimental group was given leuprorelin acetate microspheres for injection on the basis of the control group.Two groups were treated continuously for 12 months.Growth rate,predicted lifetime height,bone age growth status,HbA 1c,fasting blood glucose levels and thyroid function,insulin-like growth factor binding protein-3(IGFBP-3),insulin-like growth factor-1(IGF-1)levels,and incidence of complications were compared between the two groups.Results After 12 months of treatment,the growth rate and predicted lifetime height of two groups of children increased compared to before treatment,and the growth rate and predicted lifetime height of the experimental group were higher than those of the control group(P<0.01);The bone age of two groups of children increased compared to before treatment,but the bone age of the experimental group was lower than that of the control group(P<0.01).The bone age of the experimental group increased by(0.48±0.17)years,which was slower than the control group′s(0.96±0.14)years(t=11.938,P<0.001).There were no statistically significant difference in the levels of HbA 1c,fasting blood glucose,thyrotropin,total thyroxine,and total triiodothyronine between the two groups of children before and after treatment(P>0.05).After 12 months of treatment,the levels of IGFBP-3 and IGF-1 in two groups of children increased compared to before treatment(P<0.01),but there were no statistically significant difference in the levels between the two groups of children after treatment(P>0.05).There was no statistically significant difference in the total incidence of complications between the experimental group and the control group(6.67%vs.13.33%,χ^(2)=0.185,P=0.667).Conclusion The adjuvant treatment of adolescent ISS with leuprorelin acetate microspheres for injection has a significant effect,which is beneficial for the growth and development of children,increases the predicted lifetime height,and has no adverse effects on blood sugar and thyroid function.It can regulate serum levels of IGFBP-3 and IGF-1,and has certain safety.
