摘要
目的 探讨头颈部巨大脂肪瘤手术切除后的临床疗效。方法 选择我院2018年1月~2019年12月收治的5例头颈部巨大脂肪瘤患者的临床资料,其中颈前区2例,颈项部1例,胸锁乳突肌区2例;肿瘤最长径为9~15cm,所有患者均采用手术方法切除肿瘤,术后病检回示为脂肪瘤,术后予以加压包扎,局部换药,放置引流管等支持对症治疗,密切观察患者的预后情况。结果 5例患者肿瘤均完整切除,未出现声嘶、面瘫、呛咳等并发症,手术伤口无感染发生,术后均Ⅰ期愈合,术后随访6~15月未见复发。结论 头颈部脂肪瘤预后相对较好,手术切除是治疗脂肪瘤的主要方法,术中应避免损重要血管及神经,术后应密切随访,减少复发。
Objective To investigate the clinical effect of surgical resection of huge lipoma in head and neck.Methods The clinical data of 5 patients with huge head and neck lipoma from January 2018 to December 2019 in our hospital were selected,including 2 cases in the anterior cervical region,1 case in the neck region and 2 cases in the sternocleidomastoid muscle area.The longest diameter of the tumor was 9cm~15cm.All patients were treated with surgical resection of tumor,postoperative pathological examination showed lipoma,postoperative pressure bandage,local dressing,drainage tube and other support symptomatic treatment.The prognosis of the patients was closely observed.Results The tumors of 5 patients were completely removed,no hoarseness,facial paralysis,choking and other complications occurred,no infection occurred in the surgical wound.All patients healed in stage I after operation,and no recurrence was found during the follow-up period from 6 months to 15 months.Conclusion The prognosis of head and neck lipoma is relatively good,and surgical resection is the main method for the treatment of lipoma.The damage of important blood vessels and nerves should be avoided during operation,and close follow-up should be conducted after operation to reduce recurrence.
