摘要
介绍赵京伟教授从脾胃论治血管性痴呆的临床经验。赵京伟教授认为,脾胃虚损则运化不足,脾胃枢机不利则五脏受损,脾胃不足则痰瘀内生,蒙蔽清窍,脾胃功能失调是血管性痴呆致病的关键,临床上可分为脾胃虚衰证、心脾两虚证、脾肾亏虚证、痰蒙清窍证、痰瘀互阻证5个证型,脾胃虚衰证选用香砂六君子、四物汤、补中益气汤等;心脾两虚证选用归脾汤、人参养荣汤;脾肾亏虚证选用还少丹;痰蒙清窍证选用涤痰汤、指迷汤;痰瘀互阻证选用开心散、指迷汤、颠倒木金散、四物汤等合方加减,据证遣方,可获良效。
This paper introduces the clinical experience of Professor ZHAO Jingwei in treating vascular dementia from spleen and stomach.Professor ZHAO Jingwei believes that the deficiency of the spleen and stomach leads to insufficient transportation and transformation,the disturbance of qi movement in spleen and stomach leads to the damage of five viscera,and the insufficiency of spleen and stomach leads to endogenous phlegm stasis,which can cloud the seven orifices.The dysfunction of the spleen and stomach is the key to the disease of vascular dementia.Clinically,it can be divided into five syndrome types:spleen-stomach deficiency and failure syndrome,heart-spleen deficiency syndrome,spleen-kidney deficiency syndrome,phlegm clouding the seven orifices syndrome,and phlegm-stasis mutual obstruction syndrome.Xiangsha Liujunzi,Siwu Decoction,and Buzhong Yiqi Decoction,etc.are used for spleen-stomach deficiency and failure syndrome;Guipi Decoction and Renshen Yangrong Decoction are used for heart-spleen deficiency syndrome;Huanshao Pills is used for spleen-kidney deficiency syndrome;Ditan Decoction and Zhimi Decoction are used for phlegm clouding the seven orifices syndrome;the combination of Kaixin Powder,Zhimi Tang,Diandao Mujin Powder and Siwu Decoction can be modified for phlegm-stasis mutual obstruction syndrome.Make prescriptions based on different syndromes,so that a good effect can be obtained.
