摘要
目的:探究不同频率血液透析滤过(HDF)联合高通量血液透析(HFHD)治疗慢性肾衰竭(CRF)的效果。方法:选择2020年10月—2022年10月在扬州大学附属江都人民医院行血液透析的120例CRF患者作为研究对象,应用随机数表法将患者分为对照组(每月一次HDF联合HFHD)及观察组(每周一次HDF联合HFHD),各60例,对比两组肾功能指标[血肌酐(Scr)、血尿素氮(BUN)、血尿酸、血磷]、毒素清除率、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)]、血液净化指标[同型半胱氨酸(Hcy)、甲状旁腺激素(PTH)、β_(2)-微球蛋白(β_(2)-MG)]、营养指标[血红蛋白(Hb)、前白蛋白(PAB)、转铁蛋白(TRF)]、不良反应发生情况。结果:治疗前,两组肾功能、炎症因子、血液净化指标、营养指标比较,差异无统计学意义(P>0.05);治疗后,两组Scr、BUN、血尿酸、血磷、TNF-α、IL-6、CRP、Hcy、PTH、β_(2)-MG较治疗前均降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组Hb、PAB、TRF较治疗前均升高,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组Scr、BUN、血尿酸、血磷清除率高于对照组,不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:CRF患者接受每周一次HDF联合HFHD,可提升毒素清除率,改善肾功能、营养指标,降低炎症因子,提高血液净化效果,安全性更高。
Objective:To investigate the effect of different frequency hemodialysis(HDF)combined with high-flux hemodialysis(HFHD)in the treatment of chronic renal failure(CRF).Method:A total of 120 patients with CRF who underwent hemodialysis in Jiangdu People's Hospital Affiliated to Yangzhou University from October 2020 to October 2022 were selected as the research objects,the patients were divided into the control group(HDF once a month combined with HFHD)and the observation group(HDF once a week combined with HFHD)by random number table method,with 60 cases in each group,and the renal function indexes[serum creatinine(Scr),blood urea nitrogen(BUN),blood uric acid,blood phosphorus],toxin clearance,inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP)],blood purification indexes[homocysteine(Hcy),parathyroid hormone(PTH),β_(2)-microglobulin(β_(2)-MG)],nutritional indexes[hemoglobin(Hb),prealbumin(PAB),transferrin(TRF)],adverse reactions of two groups were compared.Result:Before treatment,there were no significant differences in renal function,inflammatory factors,blood purification indexes and nutritional indexes between two groups(P>0.05);after treatment,the Scr,BUN,blood uric acid,blood phosphorus,TNF-α,IL-6,CRP,Hcy,PTH andβ_(2)-MG in two groups were lower than those before treatment,and which in the observation group were lower than the control group,the differences were statistically significant(P<0.05);after treatment,the Hb,PAB and TRF in two groups were higher than those before treatment,and which in the observation group were higher than the control group,the differences were statistically significant(P<0.05).The clearance rate of Scr,BUN,blood uric acid and blood phosphorus in the observation group were higher than those in the control group,and the incidence of adverse reactions was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:CRF patients receiving HDF once a week combined with HFHD can increase toxin clearance rate,improve renal function and nutritional indicators,reduce inflammatory factors,improve blood purification effect,and have higher safety.
