摘要
目的 比较采用经横突-椎弓根单侧入路和经椎弓根双侧入路时经皮后凸成形术(PKP)治疗腰椎骨折(OVCF)的疗效差别。方法 选取2018年8月至2022年8月收治的60例符合纳入标准的骨质疏松性椎体压陷性骨折(OVCF)患者,分为经横突-椎弓根单侧入路组(A组,30例)和标准双侧椎弓根入路组(B组,30例)。收集两组的X线透视次数、骨水泥注入量、手术时长、术前术后疼痛量表评分数值(VAS)、骨水泥弥散率等指标,并进行组间比较。应用SPSS 18.0统计学软件对所收集数据进行分析比对后得出结果。结果 A组的手术用时、X线透视次数、骨水泥注入量数值小于B组,A组的骨水泥分布弥散率也低于B组,差异有统计学意义(P<0.05)。两组手术前后VAS评分差异经比较无统计学意义(P>0.05)。结论采用经横突-椎弓根单侧入路和经椎弓根双侧入路PKP手术治疗OVCF的疗效基本一致,但采用经横突-椎弓根单侧入路PKP的手术用时、X线辐射量、骨水泥注入量相对减少。
Objective To compare the curative effect of percutaneous kyphoplasty under the transverse process-pedicle unilateral approach and the standard bilateral pedicle approach in the treatment of lumbar osteoporotic vertebral compression fracture,and explore the difference between them.Methods From August 2018 to August 2022,a total of 60 patients with lumbar OVCF who met the inclusion criteria were selected from our hospital,and were divided into two groups according to different surgical approaches:transtransverse process-pedicle unilateral approach group(group A,30 cases) and standard bilateral pedicle approach group(group B,30 cases).Record and compare the operation fluoroscopy times,the amount of bone cement injection,the operation time,the VAS score of preoperative and postoperative pain,and the bone cement dispersion rate between the two groups.SPSS 18.0 statistical software was used for data analysis.Results The surgical time,X-ray fluoroscopy frequency,and bone cement injection volume of Group A cases were lower than those of Group B.The dispersion rste of bone cement distribution in Group A cases Was also lower than of Group B,and the difference was statistically significant(P<0.05).There was no statistically significant difference in VAS scores between the two groups of patients before and after surgery(P>0.05)Conclusions The treatment of lumbar OVCF with PKP through the transverse process-pedicle unilateral approach and the standard bilateral pedicle approach has the same effect,but the operation time is shorter,the radiation is less,and the amount of bone cement injection is less.
