摘要
随着对舒适化医疗需求的不断增加,无痛胃肠镜检查也越来越受到广大患者的认可。丙泊酚复合阿片类药物是临床上无痛胃肠镜检查的经典搭配,但丙泊酚对呼吸系统和循环系统的抑制作用较大,常危及患者的生命安全。瑞玛唑仑作为新型超短效苯二氮类药物,具有起效快、半衰期短、代谢产物无蓄积、对呼吸循环影响小且能被氟马西尼迅速逆转等优点,运用于无痛胃肠镜检查可以显著减少阿片类药物的用量、降低不良反应的发生率、缩短术后苏醒时间和提高苏醒质量,加快床位周转,为无痛胃肠镜麻醉用药提供了新方向。
With the increasing demand for comfortable medical care,painless gastrointestinal endoscopy is increasingly favored and recognized by the majority of patients.Propofol combined with opioids is the classic combination of painless gastrointestinal endoscopy in clinic.However,propofol has a great inhibitory effect on respiratory and circulatory systems,which often endangers the life safety of patients.As a new type of ultra-short-acting benzodiazepine,remimazolam has the advantages of rapid onset,short half-life,no accumulation of metabolites,little impact on respiratory and circulatory system,and can be rapidly reversed by flumazenil.The application of remimazolam in painless gastrointestinal endoscopy can significantly reduce the dosage of opioids,reduce the incidence of adverse reactions,shorten the postoperative recovery time and improve the quality of recovery,and speed up bed turnover.It provides a new direction for painless gastroenteroscopic anesthesia medication.
