呼吸康复训练联合机械振动排痰在慢性阻塞性肺疾病机械通气患者护理中的应用分析 被引量:5

摘要 目的分析呼吸康复训练联合机械振动排痰在慢性阻塞性肺疾病(COPD)机械通气患者护理中的应用效果。方法选取2021年4月至2023年3月期间于本院行无创机械通气(NPPV)治疗的COPD患者62例,使用随机数字表分为两组。对照组31例患者接受常规护理干预,研究组31例患者在其基础上应用呼吸康复训练联合机械振动排痰进行护理,干预1个月。针对两组患者的血气指标、肺功能、机械通气时间、住院时间、机械通气并发症情况、乔治呼吸疾病问卷(SGRQ)评分进行比较。结果干预后,研究组血气指标中动脉血二氧化碳分压(PaCO_(2))指标低于对照组(P<0.05),血氧饱和度(SaO_(2))、氧合指数[PaO_(2)/吸入氧浓度(FiO_(2))]、动脉血氧分压(PaO_(2))高于对照组(P<0.05)。干预后,研究组肺功能指标用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC均高于对照组(P<0.05)。研究组机械通气、住院时间均短于对照组(P<0.05)。机械通气并发症发生率,研究组低于对照组(P<0.05)。干预后,研究组SGRQ分值低于对照组(P<0.05)。结论呼吸康复训练联合机械振动排痰可以有效改善COPD机械通气患者的血气指标与肺功能,降低机械通气并发症风险,缩短康复进程,提高生活质量。 Objective To analyze the application effect of respiratory rehabilitation training combined with mechanical vibration sputum excretion in the care of patients with chronic obstructive pulmonary disease(COPD)undergoing mechanical ventilation.Methods A total of 62 COPD patients who underwent NPPV treatment in our hospital from April 2021 to March 2023 were selected and randomly divided into two groups using a random number table,with 31 patients in each group.The control group received routine nursing,while the study group received respiratory rehabilitation training combined with mechanical vibration sputum drainage based on traditional routine nursing,with each group intervention for 1 month.The blood gas indicators,lung function,mechanical ventilation time,hospital stay,mechanical ventilation complications,and George's Respiratory Disease Questionnaire(SGRQ)score between two groups were compared.Results After intervention,arterial partial pressure of carbon dioxide(PaCO_(2))in the study group was lower than that in the control group(P<0.05),while blood oxygen saturation(SaO_(2)),arterial partial pressure of oxygen(PaO_(2)),oxygenation index[PaO_(2)/inhalation oxygen concentration(FiO_(2))],and PaO_(2)were higher than those in the control group(all P<0.05).The study group had higher lung function indicators such as forced vital capacity(FVC),forced expiratory volume(FEV_(1))in 1 second,and FEV_(1)/FVC compared to the control group(all P<0.05).The mechanical ventilation and hospitalization time in the study group were shorter than those in the control group(both P<0.05).The incidence of mechanical ventilation complications was lower in the study group than in the control group(P<0.05).The SGRQ score of the study group was lower than that of the control group(P<0.05).Conclusions The combination of respiratory rehabilitation training and mechanical vibration sputum excretion can effectively improve the blood gas indicators and lung function of COPD patients with mechanical ventilation,reduce the risk of mechanical ventilation complications,shorten the rehabilitation process,and improve quality of life.
