摘要
肝性脑病(hepatic encephalopathy,HE)也称门体脑病(portosystemic encephalopathy,PSE),是一种发生于晚期肝衰竭患者中严重肝功能障碍,引起的以代谢功能紊乱为基础的复杂脑功能受损综合征。HE机制至今尚不明确。目前,随着肠道菌群的深入研究,HE与肠道菌群改变,会增加机体内毒素的产生和吸收,这种肠源性内毒素血症(intestinal endotoxermia,IETM)是诱发和加重HE的重要机制之一。因此,调节肠道菌群及改善肠道微生态系统已成为临床关注、防治HE的热点和难点之一。该文就肠道微生态概述,肠道微生态与HE的影响,通过肠道微生态干预HE的机制和作用进行综述,旨在为进一步探索HE的发病机制提供新理论依据,为临床治疗HE提供新的思路。
Hepatic encephalopathy(HE),also known as portosystemic encephalopathy(PSE),is a serious liver disorder that occurs in patients with advanced liver failure.Complex impaired brain function syndrome caused by metabolic dysfunction.The HE mechanism is still unclear.At present,with the in-depth study of intestinal flora,changes in HE and intestinal flora can increase the production and absorption of endotoxin in the body,and this intestinal endotoxemia(IETM)is one of the important mechanisms to induce and aggravate HE.Therefore,the regulation of intestinal microflora and improvement of intestinal microecosystem have become one of the hot spots and difficulties in clinical attention and prevention and treatment of HE.In this paper,it reviewed the overview of intestinal microecology,the impact of intestinal microecology on HE and the mechanism and effect of intestinal microecology on HE intervention,aiming to provide new theoretical basis for further exploration of the pathogenesis of HE and new ideas for clinical treatment of HE.
