摘要
目的分析非结核分枝杆菌(non-tuberculous mycobacteria,NTM)肺病伴肺空洞的危险因素,为更有效防治非结核分枝杆菌肺病(NTM pulmonary disease,NTM-PD)提供依据。方法回顾性分析2020年1月至2022年6月笔者科室收治的NTM-PD患者的资料,根据影像学是否合并空洞,分为有空洞组与无空洞组,收集人口学特征、临床症状、合并症、体质量指数(body mass index,BMI),白蛋白,影像学特征等资料。采用多因素Logistic回归分析NTM-PD伴肺空洞患者的危险因素。结果共纳入103例NTM-PD患者,其中合并肺空洞患者53例(51.5%),多因素Logistic回归分析显示吸烟(OR=3.732,95%CI:1.229~11.336)、BMI<20kg/m^(2)(OR=3.407,95%CI:1.178~9.854)、疾病病程≥1个月(OR=3.638,95%CI:1.161~11.396)、血清白蛋白<35g/L(OR=3.479,95%CI:1.300~9.310)、慢性阻塞性肺疾病(chronic obstructive pulmonarydisease,COPD)病史(OR=3.157,95%CI:1.035~9.637)是NTM-PD伴空洞的危险因素(P<0.0.5)。结论吸烟、BMI<20kg/m^(2)、疾病病程≥1个月、血清白蛋白<35g/L、COPD病史的NTN-PD患者更容易合并肺空洞,应及早防治。
Objective To analyze the risk factors of pulmonary disease with pulmonary cavity caused by non-tuberculous mycobacteria(NTM)and provide basis for more effective prevention and treatment of NTM pulmonary disease(NTM-PD).Methods Retrospective analysis was made on the data of NTM-PD patients admitted in our department from January 2020 to June 2022.According to whether there were cavities in imaging,they were divided into two groups:cavity group and no cavity group.Demographic characteristics,clinical symptoms,complications,body mass index,albumin,imaging characteristics and other datas were collected.Multivariate logistic regression analysis of risk factors of NTM-PD patients with pulmonary cavity.Results A total of 103 NTM-PD patients were included,including 53 patients(51.5%)with pulmonary cavity.Multivariate logistic regression analysis showed that smoking(OR=3.732,95%CI:1.229–11.336),body mass index(BMI)<20kg/m^(2)(OR=3.407,95%CI:1.178–9.854),disease course≥1 month(OR=3.638,95%CI:1.161–11.396),serum albumin<35g/L(OR=3.479,95%CI:1.300–9.310).The history of chronic obstructive pulmonary disease(COPD)(OR=3.157,95%CI:1.035–9.637)was risk factors for NTM-PD with cavities(P<0.05).Conclusion NTN-PD patients with smoking,BMI<20kg/m^(2),disease course≥1 month,serum albumin<35g/L,and COPD disease history are more likely to have pulmonary cavities,which should be prevented and treated as soon as possible.
