摘要
目的探析改良法超声内镜(endoscopic ultrasound,EUS)引导下食管胃静脉曲张(gastroesophageal varices,GOV)精准治疗的临床效果。方法选择南方医科大学顺德医院消化内镜中心2021年1月至2023年2月择期行内镜下组织胶注射的75例GOV患者,分为改良法EUS组、EUS联合普通内镜组、普通内镜组三组,每组25例。比较三组一般资料、再出血率、溃疡率等。结果三组术后3个月、6个月再出血发生率及3个月溃疡率对比,差异无统计学意义(P>0.05),但改良法EUS组3个月再出血率明显低于EUS联合普通内镜组,改良法EUS组、EUS联合普通内镜组3个月溃疡率明显低于普通内镜组,术后1个月三组再出血发生率对比,差异有统计学意义(P<0.05),且改良法EUS组、EUS联合普通内镜组明显低于普通内镜组。超声组(改良法EUS组+EUS联合普通内镜组)存在交通支26例,无交通支24例,术后1个月、6个月存在交通支与无交通支超声组患者再出血发生率对比,差异无统计学意义(P>0.05),而术后3个月存在交通支超声组再出血发生率明显高于无交通支超声组患者(P<0.05)。结论改良法EUS引导下组织胶注射应用于GOV治疗中临床效果显著,可降低再出血风险,值得临床推广。
Objective To explore the clinical efficacy of improved endoscopic ultrasound(EUS)guided precise treatment of gastroesophageal varices(GOV).Methods Seventy-five GOV patients who underwent endoscopic tissue glue injection at the Digestive Endoscopy Center,Shunde Hospital of Southern Medical University from Jan.2021 to Feb.2023 were selected.They were divided into the modified EUS group,EUS combined with conventional endoscopy group and conventional endoscopy group,with 25 patients in each group.Compare the general information,rebleeding rate,ulcer rate,etc.of 3 groups.Results There was no statistically significant difference in the incidence of rebleeding and ulcer at 3 months and 6 months after surgery among the three groups(P>0.05).However,the rebleeding rate at 3 months in the modified EUS group was significantly lower than that in the EUS combined with conventional endoscopy group,and the ulcer rate at 3 months in the modified EUS group and EUS combined with conventional endoscopy group was significantly lower than that in the conventional endoscopy group.There was a statistically significant difference in the incidence of rebleeding among the 3 groups 1 month after surgery(P<0.05),and the modified EUS group and EUS combined with conventional endoscopy group were significantly lower than the conventional endoscopy group.The ultrasound group(modified EUS group+EUS combined with ordinary endoscopy group)had 26 cases of communicating branches and 24 cases of no communicating branches.The incidence of rebleeding in patients with communicating branches and no communicating branches ultrasound group was not statistically significant at 1 month and 6 months after surgery(P>0.05),while the incidence of rebleeding in patients with communicating branches ultrasound group was significantly higher than that in patients without communicating branches ultrasound group at 3 months after surgery(P<0.05).Conclusion The improved EUS guided tissue glue injection has a significant clinical effect in the treatment of GOV,which can reduce the risk of rebleeding and it is worthy of clinical promotion.
