摘要
目的:了解天津市宁河区0~18周岁儿童青少年意外伤害发生特征,为制定意外伤害预防控制措施提供参考依据。方法:收集2018—2022年宁河区20家哨点医院的意外伤害监测病例,对数据进行统计分析。结果:共纳入10565例儿童青少年意外伤害病例,占同期意外伤害病例的19.73%,男女性别比为1.80∶1;意外伤害主要发生原因依次为跌倒(48.55%)、动物伤(26.82%)、暴力斗殴(9.19%)、交通事故(7.34%)、中毒(4.60%)、利器切割伤(3.50%);意外伤害主要发生场所依次为家中(54.44%)、学校与公共场所(17.96%)、公路/街道(13.82%)、公共居住场所(11.04%)、工业和建筑场所(1.51%)和体育运动场所(1.22%)。意外伤害发生部位依次为上肢(36.36%)、头面部(22.68%)、下肢(22.24%)、身体多部位(14.67%);意外伤害严重程度以轻度为主(81.98%),其次为中度(14.55%)、重度(3.45%)、死亡(0.02%)。不同年龄组、性别儿童青少年意外伤害的发生场所、原因、部位、严重程度差异均有统计学意义(P值均<0.01)。结论:儿童青少年意外伤害多发生在家中,上肢是意外伤害多发部位,应根据儿童青少年意外伤害的发生特点采取针对性的措施,降低意外伤害的发生率,保障儿童青少年健康成长。
OBJECTIVE To understand the characteristics of accidental injury occurrence among children and adolescents aged 0~18 years in Ninghe District of Tianjin,and to provide a reference basis for developing accidental injury prevention and control measures.METHODS Accidental injury surveillance cases were collected from 20 sentinel hospitals in Ninghe district from 2018 to 2022,and the data were analyzed.RESULTS A total of 10565 cases of accidental injuries in children and adolescents were included,accounting for 19.73%of accidental injury cases in the same period,with a male to female sex ratio of 1.80∶1.The main causes of accidental injuries occurred in the order of falls(48.55%),animal injuries(26.82%),traffic accidents(7.34%),violent fights(9.19%),poisoning(4.60%),and sharps cutting injuries(3.50%).The main locations of accidental injuries were home(54.44%),schools and public places(17.96%),highways/streets(13.82%),public housing places(11.04%),industrial and construction places(1.51%)and sports places(1.22%)in that order.The injury-prone areas were upper limbs(36.36%),head and face(22.68%),lower limbs(22.24%),and multiple body parts(14.67%),in that order.The severity of accidental injuries was mainly mild(81.98%),moderate(14.55%),severe(3.45%),and fatal(0.02%).Except for no statistically significant differences in accidental injury sites among children and adolescents of different genders,there were statistically significant differences in the location,cause,severity and site of injury among different age groups and genders(P<0.01).CONCLUSION Injuries mostly occur at home,and the upper limbs are the most frequent sites.Targeted measures should be taken according to the characteristics of injury occurrence among adolescent children to reduce the incidence of injury and maintain the healthy growth of children.
