ICU护士道德困扰现状及其影响因素分析

The status quo of moral distress in ICU nurses and its influencing factors

摘要 目的调查医院ICU护士的道德困扰现状,并分析其影响因素。方法对210名ICU护士采用一般资料调查表、道德困扰量表、离职意愿量表、工作场所暴力量表进行调查。结果不同年龄、文化程度、工作年限、1年内遭受工作场所暴力频次的ICU护士,其道德困扰量表得分比较,差异有统计学意义(P<0.01)。ICU护士道德困扰量表得分为(2.76±0.21)分,离职意愿量表得分为(2.89±0.43)分,工作场所暴力量表得分为(6.98±1.89)分。ICU护士道德困扰与护士离职意愿、工作场所暴力呈正相关(r=0.471、0.552,P<0.01)。多重线性回归分析结果显示,文化程度、工作年限、工作场所暴力、离职意愿均是ICU护士道德困扰的影响因素,共可解释道德困扰总体41.80%的变异。结论ICU护士道德困扰水平较高,管理者应采取积极的干预措施,例如减少工作场所暴力等方法来降低ICU护士道德困扰水平。 Objective To investigate the status quo of moral distress in ICU nurses and analyze influencing factors.Methods Surveys were conducted in 210 ICU nurses using general information questionnaire,Moral Distress Scale(MDS),Turnover Intention Scale(TIS)and Workplace Violence Scale(WVS).Results Differences were statistically significant in the MDS scores among ICU nurses with different ages,educational levels,working years and frquency of workplace violence within 1 year(P<0.01).The MDS score was(2.76±0.21),TIS(2.89±0.43),and WVS(6.98±1.89).Moral distress of ICU nurse was positively correlated with turnover intention and workplace violence(r=0.471,0.552;P<0.01).Multiple linear regression analysis showed that education levels,working years,workplace violence and turnover intention were the factors influencing moral distress of ICU nurses,could explain 41.80% of the total variation of moral distress altogether.Conclusion Moral distress level of ICU nurses is higher and active intervention measures such as reducing workplace violence etc.should be adopted to reduce moral distress level.
