摘要
目的探讨艾滋病(AIDS)确诊后CD_(4)^(+)/CD_(8)^(+)动态的变化,以及抗病毒干预的影响。方法选取2017年1月—2019年12月揭阳市疾控中心确诊的AIDS患者950例。收集研究对象确诊后以及首次抗病毒治疗1周、1、6、12个月时的CD_(4)^(+)、CD_(8)^(+)淋巴细胞计数以及性别、年龄等基线资料。使用秩和检验的统计学方法对其CD_(4)^(+)、CD_(8)^(+)淋巴细胞计数水平以及CD_(4)^(+)/CD_(8)^(+)比值变化进行动态描述,并分析其原因。结果基线CD_(4)^(+)细胞数在0~300个/μL的病例,其治疗后第12个月CD_(4)^(+)/CD_(8)^(+)比值明显低于基线CD_(4)^(+)细胞数>300个/μL的病例,差异有统计学意义(P<0.05)。其他基线数据对治疗后第12个月CD_(4)^(+)/CD_(8)^(+)比值影响差异无统计学意义(P>0.05)。与治疗前的基线CD_(4)^(+)/CD_(8)^(+)比值相比,自治疗第1个月开始,CD_(4)^(+)/CD_(8)^(+)比值提升,差异有统计学意义(P<0.05)。随治疗时间延长,CD_(4)^(+)/CD_(8)^(+)比值≥1构成比呈现上升趋势。结论治疗1个月后,HIV患者CD_(4)^(+)/CD_(8)^(+)比值水平开始有较为明显的提升。该比值水平的提升与患者基线CD_(4)^(+)水平有关。基线CD_(4)^(+)T淋巴细胞水平可能成为预测CD_(4)^(+)/CD_(8)^(+)比值变化及患者预后的辅助手段。
Objective To investigate the dynamic changes of CD_(4)^(+)/CD_(8)^(+)after AIDS confirmed diagnosis and the effect of antiviral intervention.Methods A total of 950 AIDS patients confirmed by the Jieyang Center for Disease Control and Prevention from January 2017 to December 2019 were selected.The CD_(4)^(+)and CD_(8)^(+)T lymphocyte counts of the study subjects after diagnosis and at 1 week,1 month,6 months and 12 months of initial antiviral treatment were selected,as well as baseline data such as gender and age.The changes in CD_(4)^(+)and CD_(8)^(+)T lymphocyte count levels,and CD_(4)^(+)/CD_(8)^(+)ratio were dynamically described using statistical methods such as rank sum test,and the reasons for these changes were analyzed.Results The cases with a baseline CD_(4)^(+)cell count of 0-300 cells/μL had significantly lower CD_(4)^(+)/CD_(8)^(+)ratios at 12th month after treatment compared to cases with a baseline CD_(4)^(+)cell count over 300 cells/μL,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the impact of other baseline data on the CD_(4)^(+)/CD_(8)^(+)ratio at the 12th month after treatment(P>0.05).Compared with the baseline CD_(4)^(+)/CD_(8)^(+)ratio before treatment,the CD_(4)^(+)/CD_(8)^(+)ratio increased from the first month of treatment,and the difference was statistically significant(P<0.05).With the prolongation of treatment time,the proportion of CD_(4)^(+)/CD_(8)^(+)1 showed an upward trend.Conclusion After one month of treatment,the CD_(4)^(+)/CD_(8)^(+)ratio level of HIV patients began to show a significant increase.The increase in this ratio level was related to the patient's baseline CD_(4)^(+)level.Baseline CD_(4)^(+)T lymphocyte levels might become an auxiliary tool for predicting changes in CD_(4)^(+)/CD_(8)^(+)ratio and patient prognosis.
