摘要
目的探讨分析微创消融术联合枳壳甘草汤治疗腰椎间盘突出的临床效果。方法选取2021年7月至2022年12月在揭阳市揭东区第二人民医院治疗的腰椎间盘突出患者60例作为本次研究对象,以数字表法随机将其分为两组,各30例,所有纳入患者均接受微创消融术治疗,术后治疗A组接受常规干预治疗,治疗B组则在A组治疗的基础上加用枳壳甘草汤治疗。观察并对比两组受试对象的临床疗效、疼痛程度、腰椎功能、血清MDA、SOD水平变化以及炎症水平变化。结果治疗B组应用微创消融术联合枳壳甘草汤治疗干预后的临床有效率较治疗A组更为显著(P<0.05);在术后3个月两组患者的VAS评分较干预前均有了明显改善(P<0.05),在术后第3个月治疗B组患者的VAS评分显著优于治疗A组(P<0.05);干预后两组患者的腰椎功能较干预前均明显改善(P<0.05),且干预后治疗B组的腰椎ODI评分显著优于治疗A组(P<0.05);干预前和干预3个月后两组患者的血清MDA、SOD水平差异均无统计学意义(P>0.05);干预3个月后,两组患者的炎症因子水平较干预前无明显变化(P>0.05),且干预3个月后两组患者的IL-6水平差异无统计学意义(P>0.05),但治疗B组患者的TNF-α水平显著低于治疗A组(P<0.05)。结论应用微创消融术联合枳壳甘草汤治疗腰椎间盘突出患者的临床有效性确切,其可有效增强患者的腰椎功能,减轻其疼痛程度,并改善其TNF-α水平,值得临床推荐。
Objective To investigate the clinical effect of minimally invasive ablation combined with Zhiqiao Gancao decoction in the treatment of lumbar disc herniation.Methods 60 patients with lumbar disc herniation treated in Jiedong District Second People's Hospital of Jieyang City from July 2021 to December 2022 were selected as the subjects of this study.They were randomly divided into two groups by digital table method,30 cases in each group.All patients were treated with minimally invasive ablation.After operation,group A received routine intervention treatment,while group B received Zhiqiao Gancao Decoction on the basis of group A treatment.The clinical efficacy,pain degree,lumbar function,serum MDA,SOD level and inflammation level of the two groups were observed and compared.Results The clinical effective rate of minimally invasive ablation combined with Zhiqiao Gancao Decoction in group B was more significant than that in group A(P<0.05).3 months after operation,the VAS scores of the two groups were significantly improved compared with those before intervention(P<0.05).3 months after operation,the VAS score of group B was significantly better than that of group A(P<0.05).After intervention,the lumbar function of the two groups was significantly improved compared with that before intervention(P<0.05),and the lumbar ODI score of the treatment group B was significantly better than that of the treatment group A(P<0.05).There was no significant difference in serum MDA and SOD levels between the two groups before and after 3 months of intervention(P>0.05).After 3 months of intervention,the levels of inflammatory factors in the two groups were not significantly different from those before intervention(P>0.05),and there was no significant difference in IL-6 levels between the two groups after 3 months of intervention(P>0.05),but the level of TNF-αin the treatment group B was significantly lower than that in the treatment group A(P<0.05).Conclusion The clinical efficacy of minimally invasive ablation combined with Zhiqiao Gancao Decoction in the treatment of patients with lumbar disc herniation is definite.It can effectively enhance the lumbar function of patients,reduce the degree of pain,and improve the level of TNF-α,which is worthy of clinical recommendation.
