摘要
目的报告应用1.5mm锁定加压接骨板(locking compression plate,LCP)双平面固定近节指骨粉碎性骨折的临床疗效。方法自2018年1月至2021年12月,我们采用1.5mm LCP双平面固定治疗近节指骨粉碎性骨折患者15例。分别于术后4、12.24周复查X线片,评估骨折愈合情况,并测量患者健侧与患侧关节主动活动范围(range of motion,ROM)。末次随访采用手指总主动活动度(total activemotion,TAM)法评定功能。结果术后所有患者均获得随访,时间为6~18个月,平均(9.3±2.4)个月,所有骨折均达到解剖复位,骨折愈合时间为1.5~3.0个月,平均(2.1±1.1)个月。患侧与健侧掌指关节、指间关节活动范围比较,差异无统计学意义(P>0.05)。末次随访手指TAM法评定功能:优11例,良2例,中2例。所有患者均未出现骨折不愈合、移位、功能障碍、畸形。结论应用1.5mmLCP双平面固定近节指骨粉碎性骨折,骨折固定强度高,伸肌腱干扰小,利于患者术后早期康复锻炼,可获得满意的疗效。
Objective To report the clinical efficacy of 1.5 mm locking compression plate(LCP)for biplane fixation of comminuted proximal phalangeal fractures.Methods From January 20i8 to December 2021,15 patients with comminuted proximal phalangeal fractures were treated by 1.5 mm LCP biplane fixation.X-ray films were reviewed at 4,12,and 24 weeks after surgery to assess fracture healing and measure the range of motion(ROM)of the healthy and affected sides of the patient's joints.At the last follow-up,total active motion(TAM)was used to assess finger function.Results All the patients were follow-up for 6 to 18 months,with an average of(9.3±2.4)months.All the fractures achieved anatomical reduction.The fracture healing time was 1.5 to 3.0 months,with an average of(2.1±1.1)months.There was no significant difference in the range of motion of the metacarpophalangeal and interphalangeal joints between the affected and healthy sides(P>0.05).At the last follow-up,according to TAM standard,the functional results were rated as excellent in il cases,good in 2 cases,and fair in 2 cases.All the patients had no nonunion,displacement,dysfunction,or deformity of the fracture.ConclusionThe application of 1.5 mm LCP biplane fixation for comminuted proximal phalangeal fractures has a high fixation strength and low extensor tendon interference,which is conducive to early postoperative rehabilitation and exercise for patients,and can achieve satisfactory results.
