摘要
甘麦大枣汤是《金匮要略》治疗脏燥证的名方,历代医家从不同角度对其阐发,其中叶天士留存相关医案最为丰富且颇有发挥。针对脏躁病机,叶氏从调补心脾以使中宫有恃的角度阐释,为治疗情志病证提供补脾益心镇怯的思路。叶天士“仿经义”“宗仲景”,基于《素问·脏气法时论篇》“肝苦急,急食甘以缓之”的理论,揭示甘麦大枣汤蕴含“甘缓”大法,指出具有镇肝安胃、和阳熄风以及燮变阴阳失衡的作用,进而拓展其临证运用,以调治肝胃失和、心肾不交、肝体不足而肝用太过、阴伤失摄而阳不下潜等病证,并将甘麦大枣汤与其他经方结合运用,为后世认识与运用甘麦大枣汤开拓思路,可谓是中医学守正创新的典范。
Ganmai Dazao decoction is a famous prescription for treating visceral dryness syndrome in“The Synopsis of the Golden Chamber”.Doctors of the past dynasties have analyzed it from different angles.Among them,YE Tianshi has the most abundant and useful medical records.In view of the pathogenesis of Zangzang,YE explained it from the perspective of tonifying the heart and spleen to make the middle palace strong,and provided ideas for tonifying the spleen,benefiting the heart and calming timidity for the treatment of emotional and mental diseases.YE Tianshi’s“imitating the meaning of the classics”and“ZONG Zhongjing”,based on the theory of“Suwen Zangqi Fashi Lun”that“the liver is painful and urgent,eat sweet food quickly to relieve it”,revealed that Ganmai Dazao decoction contains the“sweet and slowing”method,pointed out that it has the functions of calming the liver and stomach,calming yang and calming wind,and changing the imbalance of yin and yang,and then expanded its clinical application to regulate disharmony of liver and stomach,disharmony of heart and kidney,deficiency of liver body and excessive use of liver,and yin injury.The combination of Ganmai Dazao decoction with other classic prescriptions has opened up new ideas for future generations to understand and apply Ganmai Dazao decoction.It can be said to be a model of integrity and innovation in traditional Chinese medicine.
