摘要
目的分析全麻手术患者麻醉维持中应用不同剂量环泊酚复合七氟烷的临床效果。方法选择2021年10月—2022年12月徐州市肿瘤医院收治的80例全麻手术患者作为研究对象,以术中环泊酚应用剂量划分为两组,每组40例。低剂量组采取0.4 mg/(kg·h)环泊酚复合七氟烷麻醉,高剂量组采取0.9 mg/(kg·h)环泊酚复合七氟烷麻醉。对比两组患者围术期指标、血流动力学指标、简明精神状态量表评分、Ramsay镇静评分及不良反应。结果两组患者手术、麻醉时间比较,差异无统计学意义(P>0.05);高剂量组术后苏醒、拔管时间分别为(17.22±3.42)、(34.10±5.36)min,均长于低剂量组的(11.83±1.34)、(22.31±4.25)min,差异有统计学意义(t=9.281、10.901,P<0.05)。T0、T1、T2时刻两组患者平均动脉压、心率比较,差异无统计学意义(P>0.05)。高剂量组简明精神状态量表评分低于低剂量组,Ramsay镇静评分高于低剂量组,差异有统计学意义(P<0.05)。高剂量组不良反应发生率为20.00%,高于低剂量组的5.00%,差异有统计学意义(χ^(2)=4.114,P<0.05)。结论全麻手术患者麻醉维持中应用不用剂量环泊酚复合七氟烷均可维持血流动力学稳定性;但小剂量环泊酚可减轻患者认知功能受损程度,用药安全性更高;而高剂量环泊酚镇静效果更佳。
Objective To analyze the clinical effect of different doses of Ciprofol combined with Sevoflurane in anes-thesia maintenance of patients undergoing general anesthesia.Methods A total of 80 patients undergoing general anes-thesia treated in Xuzhou Cancer Hospital from October 2021 to December 2022 were selected as the study objects,and were divided into two groups according to the applied dosage of intraoperative pofol,with 40 cases in each group.The low dose group was anesthetized with 0.4 mg/(kg·h)cypofol combined with Sevoflurane,and the high dose group was anesthetized with 0.9 mg/(kg·h)cypofol combined with Sevoflurane.Perioperative indexes,hemodynamic in-dexes,concise mental state scale score,Ramsay sedation score and adverse reactions were compared between the two groups.Results There was no statistically significant difference in operation and anesthesia time between the two groups(P>0.05).The time of recovery and extubation in the high-dose group were(17.22±3.42)min and(34.10±5.36)min,respectively,which were longer than those in the low-dose group(11.83±1.34)min and(22.31±4.25)min,and the difference was statistically significant(t=9.281,10.901,P<0.05).There was no statistically significant differ-ence in mean arterial pressure and heart rate between the two groups at T0,T1 and T2(P>0.05).The score of concise mental state scale in high-dose group was lower than that in low-dose group,and the score of Ramsay sedation was higher than that in low-dose group,the difference was statistically significant(P<0.05).The incidence of adverse reac-tions in the high-dose group was 20.00%,higher than that in the low-dose group(5.00%),and the difference was sta-tistically significant(χ^(2)=4.114,P<0.05).Conclusion The hemodynamic stability of patients undergoing general anes-thesia can be maintained by using Ciprofol combined with Sevoflurane without dose.However,low dose Ciprofol can reduce the degree of cognitive impairment in patients,and the drug safety is higher.High dose of Ciprofol has better sedative effect.
