摘要
目的研究不同射频消融术式对预激综合征伴心房颤动患者的临床疗效。方法回顾性分析2020年2月—2023年3月盐城市第三人民医院(南京医科大学盐城临床医学院)收治的91例预激综合征伴心房颤动患者的临床资料。根据消融术式不同将患者分为两组,其中观察组46例,对照组45例。观察组行环肺静脉隔离术(catheter-based pulmonary vein isolation,CPVI)联合左心房顶部线性阻滞(left atrial roof ablation,LARA)治疗,对照组行CPVI治疗。观察并比较两组患者手术时间及X线曝光时间,出院后随访,对比两组患者消融成功率、预激综合征复发率及术后并发症发生率。结果两组患者手术时间及X线曝光时间对比,差异无统计学意义(P>0.05)。观察组消融成功率(95.65%)高于对照组(82.22%),差异有统计学意义(χ^(2)=4.195,P=0.041)。观察组预激综合征复发率(2.17%)低于对照组(17.78%),差异有统计学意义(χ^(2)=6.215,P=0.013)。两组患者术后并发症发生率对比,差异无统计学意义(χ^(2)=0.236,P=0.627)。结论LARA与CPVI联合治疗预激综合征伴心房颤动患者,能够提高消融成功率,降低预激综合征复发率,同时,手术时间、X线曝光时间及术后并发症发生率不存在明显升高。
Objective To study the clinical effect of different radiofrequency ablation methods on patients with preex-citation syndrome with atrial fibrillation.Methods The clinical data of 91 patients with preexcitation syndrome with atrial fibrillation admitted to Yancheng Third People's Hospital(Yancheng Clinical College of Nanjing Medical Uni-versity)from February 2020 to March 2023 were retrospectively analyzed.The patients were divided into two groups according to different ablation methods,including 46 patients in the observation group and 45 patients in the control group.The observation group underwent catheter-based pulmonary vein isolation(CPVI)combined with left atrial roof ablation(LARA),while the control group underwent CPVI.The operation time and X-ray exposure time of the two groups were observed and compared.The success rate of ablation,the recurrence rate of pre-excitation syndrome and the incidence of postoperative complications were compared after discharge.Results There was no statistically signifi-cant difference in operation time and X-ray exposure time between the two groups(P>0.05).The success rate of abla-tion in observation group(95.65%)was higher than that in control group(82.22%),and the difference was statistically significant(χ^(2)=4.195,P=0.041).The recurrence rate of preexcitation syndrome in observation group(2.17%)was lower than that in control group(17.78%),and the difference was statistically significant(χ^(2)=6.215,P=0.013).There was no statistically significant difference in the incidence of postoperative complications between the two groups(χ^(2)=0.236,P=0.627).Conclusion The combination of LARA and CPVI in the treatment of patients with preexcitation syn-drome with atrial fibrillation can improve the success rate of ablation and reduce the recurrence rate of preexcitation syndrome.Meanwhile,the operation time,X-ray exposure time and postoperative complications do not significantly in-crease.
