摘要
目的探讨硬膜外分娩镇痛对初产妇产程和妊娠结局的影响。方法选取80名初产妇,依据随机抽签法分为研究组和对照组,各40名。两组均临产前予以常规护理,对照组予以自然分娩方式进行分娩,研究组在对照组基础上行硬膜外分娩镇痛。比较两组第一产程、第二产程时间、不良妊娠结局及新生儿Apgar评分。结果研究组第一产程、第二产程时间均短于对照组(P<0.05)。研究组阴道分娩率87.50%,高于对照组的67.50%,剖宫产率12.50%低于对照组的32.50%(P<0.05)。两组不良妊娠结局发生率比较,差异无统计学意义(P>0.05)。两组新生儿出生后5min、10min的Apgar评分高于出生后1min(P<0.05),其中研究组新生儿出生后1min、5min、10min的Apgar评分均高于对照组(P<0.05)。结论硬膜外分娩镇痛对初产妇具有积极意义,可缩短第一产程、第二产程时间,改善产妇分娩结局以及提高新生儿Apgar评分,值得临床推广。
Objective To investigate the effect of epidural analgesia on the labor process and pregnancy outcome of primipara.Methods 80 primiparas were selected and divided into study group and control group with 40 each according to random drawing method.Both groups were given routine care before delivery,the control group was given natural delivery,and the study group was given epidural analgesia on the basis of the control group.The first stage of labor,the second stage of labor,adverse pregnancy outcome and neonatal Apgar score were compared between the two groups.Results The first and second stages of labor in the study group were shorter than those in the control group(P<0.05).The vaginal delivery rate of the study group was 87.50%,higher than that of the control group(67.50%),and the cesarean section rate of 12.50%was lower than that of the control group(32.50%)(P<0.05).There was no statistically significant difference in the incidence of adverse pregnancy outcomes between the two groups(P>0.05).The Apgar score 5 min and 10 min after birth in the two groups was higher than that 1 min after birth(P<0.05),and the Apgar score 1 min,5 min and 10 min after birth in the study group was higher than that in the control group(P<0.05).Conclusion Epidural labor analgesia has positive significance for primipara,can shorten the time of the first and second stages of labor,improve the outcome of childbirth and increase the neonatal Apgar score,and is worthy of clinical promotion.
