拉曼光谱技术在早期肺癌诊断中的应用及其研究进展 被引量:1

The application and research status of Raman technology in the diagnosis of early lung cancer

摘要 肺癌是当前世界上发病率和致死率增长最快,对人群健康和生命威胁最大的恶性肿瘤之一。患者一旦被确诊为肺癌,往往已经处于肺癌晚期,晚期肺癌患者的五年存活率仅为18%,伴有远处转移的患者仅为5%,而Ⅰ期非小细胞肺癌患者五年存活率最高达77.9%,因此随着检测技术的快速发展和医疗水平的不断提高,早期肺癌诊断成为提高肺癌治疗效果的关键。拉曼光谱技术通过采集空间中每个像素点处的拉曼光谱信息,将分子信息在空间上展现,并定性、定量与定位地分析物质分子,以图像形式观测物质成分与结构等信息,准确地揭示生物组织的生化特性。作为一种非侵入性的检测技术,拉曼光谱技术可以实现对癌变组织与正常组织之间分子水平结构差异的检测,因此对于无症状但组织已有变化的早期肺癌临床诊断有信息丰富、制样简单、灵敏度高、准确度高等优势。利用拉曼光谱技术对肺组织切片、肺细胞和其他样本进行检测,能够在微观层面观测到组织细胞内蛋白质、脂质、核苷酸等生物分子成分的改变,可以实现对肺癌定性定量的诊断。本文综述了近年来拉曼光谱技术基于不同肺癌检测样本在实现肺癌诊断中的应用及其研究进展,并对未来拉曼光谱技术用于辅助肺癌的早期治疗进行了展望,以期为临床医生和研究者提供参考。 Lung cancer is currently one of the fastest growing malignancies in the world in terms of incidence and mortality,and is one of the most threatening to the health and lives of people.The five-year survival rate for patients with advanced lung cancer is only 18%,and only 5%for patients with distant metastases,while the five-year survival rate for stage I non-small cell lung cancer patients is as high as 77.9%.With the rapid development of detection technology and the continuous improvement of medical level,early diagnosis of lung cancer has become the key to improving the effectiveness of lung cancer treatment.Raman spectroscopy presents molecular information spatially by collecting Raman spectral information at each pixel in space and analyses the molecules of a substance qualitatively,quantitatively and locally.It provides an accurate picture of the biochemical properties of biological tissues by observing the composition and structure of a substance in image form.As a non-invasive technique,Raman spectroscopy allows the detection of structural differences between cancerous and normal tissues at the molecular level,and is therefore informative,simple,sensitive and accurate for the clinical diagnosis of asymptomatic but altered lung cancer.The use of Raman spectroscopy for the detection of lung tissue sections,lung cells and other samples enables the observation of changes in biomolecular components such as proteins,lipids and nucleotides within the tissue cells at the microscopic level,enabling the qualitative and quantitative diagnosis of lung cancer.This essay reviews the recent applications of Raman spectroscopy in achieving lung cancer diagnosis based on different lung cancer detection samples and its research progress,and provides an outlook on the future use of Raman spectroscopy to assist in the early treatment of lung cancer,with a view to providing a reference for clinicians and researchers.
