摘要
目的:探讨利伐沙班在经外周静脉穿刺的中心静脉导管(PICC)术后血栓形成中的预防效果。方法:选择2020年3月—2021年12月江门市中心医院胸外科、肿瘤科住院的PICC置管患者180例为研究对象,根据随机对照原则分为对照组(n=60,不服用抗凝药)、低剂量组(n=60,口服5 mg伐沙班,每天1次)和高剂量组(n=60,口服10 mg利伐沙班,每天1次)。比较两组D-二聚体(D-D)、肝肾功能、导管通畅率及出血事件发生情况。结果:高剂量组、低剂量组术后2周D-D水平均低于对照组,高剂量组D-D水平低于低剂量组(P<0.05);三组术前、术后当天及术后2周肝肾功能比较,差异均无统计学意义(P>0.05);三组出血发生率比较,差异无统计学意义(P>0.05);三组导管通畅率比较,差异有统计学意义,且高剂量组导管通畅率高于对照组(P<0.05)。结论:利伐沙班能预防PICC术后血栓形成,改善患者凝血功能,且对肝肾功能影响较小,可提高导管通畅率。
Objective:To explore the preventive effect of Rivaroxaban on thrombosis after peripherally inserted central venous catheter(PICC)operation.Method:A total of 180 patients with placement of tube with PICC admitted to the Department of Thoracic Surgery and Oncology of Jiangmen Central Hospital from March 2020 to December 2021 were selected as the study objects,and were divided into control group(n=60,not taken anticoagulant drug),low-dose group(n=60,oraled 5 mg Rivaroxaban,once a day)and high-dose group(n=60,oraled 10 mg Rivaroxaban,once a day)according to the randomized control principle.D-dimer(D-D),liver and kidney function,catheter patency rate and hemorrhage events were compared between the two groups.Result:2 weeks after operation,the levels of D-D in high-dose group and low-dose group were lower than that in control group,and the level of D-D in high-dose group was lower than that in low-dose group(P<0.05).There were no significant differences in liver and kidney function between the three groups before operation,on the day after operation and 2 weeks after operation(P>0.05).There was no significant difference in the incidence of hemorrhage among the three groups(P>0.05).The catheter patency rate of the three groups was statistically significant,and the catheter patency rate of the high-dose group was higher than that of the control group(P<0.05).Conclusion:Rivaroxaban can prevent thrombosis after PICC operation,improve coagulation function of patients,and have little impact on liver and kidney function,and can increase vascular patency rate.
