摘要
目的探讨腹横肌平面阻滞联合舒芬太尼、纳布啡自控镇痛在剖宫产术后镇痛中的应用效果。方法选择2022年1月至2023年6月泰安市妇幼保健院收治的80例剖宫产术后接受静脉镇痛的产妇为研究对象,按照随机数字表法分为两组,各40例。对照组术后给予舒芬太尼与纳布啡自控镇痛,观察组在对照组的基础上采用超声引导下腹横肌平面阻滞术,比较两组疼痛程度(NRS)、血清致痛物质P物质(SP)水平、并发症及镇痛泵按压次数。结果观察组术后6、24、48 h NRS评分低于对照组(P<0.05);观察组术后不同时间点NRS评分比较,差异有统计学意义(P<0.05)。观察组术后6、24、48 h SP水平低于对照组(P<0.05);观察组术后不同时间点SP水平比较,差异有统计学意义(P<0.05)。观察组皮肤瘙痒、恶心呕吐、头晕头痛、嗜睡及尿潴留总发生率低于对照组(P<0.05);观察组镇痛泵按压次数少于对照组(P<0.05)。结论腹横肌平面阻滞联合舒芬太尼、纳布啡自控镇痛在剖宫产术后镇痛中应用,可有效缓解术后疼痛,降低SP水平、并发症发生率。
Objective To investigate the application effect of transversus abdominis plane block combined with Sufentanil and Nalbuphine controlled analgesia in postoperative analgesia after cesarean section.Methods A total of 80 parturients who received intravenous analgesia after cesarean section in Tai’an Maternal and Child Health Hospital from January 2022 to June 2023 were selected as the research objects.According to the random number table method,they were divided into two groups,with 40 cases in each group.The control group was given Sufentanil and Nalbuphine controlled analgesia after operation,and the observation group was given ultrasound-guided transversus abdominis plane block on the basis of the control group.The pain degree(NRS),the level of serum pain-causing substance P(SP),complications and the number of analgesic pump compression were compared between the two groups.Results The NRS score of the observation group was lower than that of the control group at 6,24 and 48 h after operation(P<0.05).There were statistically significant differences in NRS scores at different time points after operation in the observation group(P<0.05).The SP level of the observation group was lower than that of the control group at 6,24 and 48 hours after operation(P<0.05).There were statistically significant differences in SP levels at different time points after operation in the observation group(P<0.05).The total incidence of pruritus,nausea and vomiting,dizziness and headache,drowsiness and urinary retention in the observation group was lower than that in the control group(P<0.05).The number of analgesic pump compression in the observation group was fewer than that in the control group(P<0.05).Conclusion The application of transversus abdominis plane block combined with Sufentanil and Nalbuphine controlled analgesia in postoperative analgesia after cesarean section can effectively relieve postoperative pain,reduce SP level and complication rate.
