摘要
目的探讨外周血降钙素原(PCT)、中性粒细胞(PMN)、NK细胞水平对急性脑梗死(ACI)患者预后情况的评估效能。方法选取2022年1月—2022年12月医院收治的90例ACI患者作为ACI组,并纳入同期进行体检的90名健康者作为对照组,检测两组PCT、PMN、NK细胞水平。根据出院3个月后改良Rankin量表(mRS)评分结果,将其分为预后良好组(n=67)和预后不良组(n=23),并比较2组患者PCT、PMN、NK细胞水平,采用ROC曲线分析PCT、PMN、NK细胞水平对ACI患者预后情况的评估效能。结果ACI组PCT、PMN水平高于对照组,NK细胞水平低于对照组,差异有统计学意义(P<0.05);预后良好组PCT、PMN水平低于预后不良组,NK细胞水平高于预后不良组(P<0.05);PCT、PMN以及NK细胞三者联合ROC曲线下面积(AUC)高于单独检测(P<0.05)。结论外周血PCT、PMN以及NK细胞可以用于ACI患者预后情况的辅助诊断指标,ACI患者预后情况越好,PCT、PMN水平越低,NK细胞水平越高。
Objective To explore the evaluation efficiency of peripheral blood procalcitonin(PCT),neutrophil(PMN)and NK cells for prognosis of patients with acute cerebral infarction(ACI).Methods 90 patients with ACI and 90 healthy controls in the hospital were enrolled as ACI group and control group between January and December 2022,respectively.The levels of PCT,PMN and NK cells in the two groups were detected.According to scores of modified Rankin scale(mRS)at 3 months after discharge,ACI patients were divided into good prognosis group(n=67)and poor prognosis group(n=23),and levels of PCT,PMN and NK cells in the two groups were compared.The evaluation efficiency of PCT,PMN and NK cells for prognosis of ACI patients was analyzed by ROC curves.Results The levels of PCT and PMN in ACI group were higher than those in control group,while level of NK cells was lower than that in control group(P<0.05).The level of PCT and PMN in good prognosis group were lower than those in poor prognosis group,while level of NK cells was higher than that in poor prognosis group(P<0.05).The area under the ROC curve(AUC)of PCT combined with PMN and NK cells for evaluating prognosis was greater than that of single index(P<0.05).Conclusion Peripheral blood PCT,PMN and NK cells can be applied as auxiliary diagnostic indexes for prognosis of ACI patients.In ACI patients with better prognosis,levels of PCT and PMN are lower,and level of NK cells is higher.
