Er:YAG激光用于牙周非手术治疗的短期疗效分析

摘要 目的观察Er:YAG激光用于牙周非手术治疗对中重度慢性牙周炎的短期临床效果。方法选择中、重度慢性牙周炎患者24例,共96个四分区,464颗牙,1412个位点,采用分口设计随机分为2组,实验组:应用Er:YAG激光;对照组:使用传统器械。治疗前以及治疗后6、12、24周分别记录菌斑指数、牙龈指数、探诊深度、附着丧失及探诊出血的阳性位点率,并进行统计学分析。结果两组术后6、12、24周结果显示,菌斑指数、牙龈指数、探诊深度、附着丧失和探诊出血情况均有改善(P<0.05)。中等深度的牙周袋组(4~6 mm),在术后相应的时间点两种治疗方案无显著性差异;而深袋组(≥7 mm),Er:YAG激光治疗的方案,改善探诊深度和附着丧失具有更强的优势,差异具有统计学意义(P<0.05)。结论对中、重度慢性牙周炎患者的深牙周袋单独应用Er:YAG激光治疗较传统治疗短期临床效果更佳,Er:YAG激光可以单独应用于牙周病的非手术治疗,但远期疗效仍需进一步研究。 Objective To observe the short-term clinical effect of Er:YAG laser in periodontal non-surgical treatment of moderate and severe chronic periodontitis.Methods A total of 24 patients with moderate or severe chronic periodontitis,with 96 quadripartite regions,464 teeth and 1412 sites,were randomly divided into 2 groups by mouth division design.The experimental group was treated with Er:YAG laser.And control group was treated with traditional instruments.The positive site rates of plaque index,gingival index,probing depth,attachment loss and probing bleeding were recorded before treatment and 6,12 and 24 weeks after treatment,and were statistically analyzed.Results The results of 6 weeks,12 weeks and 24 weeks after surgery showed that the plaque index,gingival index,probing depth,attachment loss and probing bleeding were improved(P<0.05).In the moderate depth periodontal pocket group(4~6mm),there was no significant difference between the two treatments at the corresponding time point after surgery.In the deep pocket group(≥7mm),Er:YAG laser treatment had a stronger advantage in improving the depth of investigation and loss of attachment,and the difference was statistically significant(P<0.05).Conclusions Er:YAG laser alone has better short-term clinical effect than traditional treatment on deep periodontitis in patients with moderate and severe chronic periodontitis.Er:YAG laser can be applied to the non-surgical treatment of periodontal disease alone,but the long-term efficacy still needs further study.
