超声心动图观察二尖瓣及肺静脉前向血流速度与肺静脉反流速度相关性的意义 被引量:2

Significance of the relationship between mitral inflow velocity patterns, pulmonary venous forward flow velocities and flow velocity of atrial reversal recorded by transthoracic Doppler echocardiography

摘要 目的 探讨二尖瓣及肺静脉前向血流速度与肺静脉反流速度相关性的意义。方法 对随机选取的 1 2 0例不同级别的左室舒张功能对象进行经胸多普勒超声心动图检查 ,并测量其二尖瓣E峰及A峰最大流速 (E、A)、肺静脉收缩期最大流速 (S)、舒张早期最大流速 (D)及反流最大流速 (PA) ,然后 ,对后者做组间比较。同时 ,对Ⅰ期左室舒张功能不全患者进行E、A、S、D、E/A值与PA的相关性分析。结果 左室舒张功能不全Ⅰ期与正常组比 ,Ⅱ期与Ⅰ期比 ,Ⅲ期与Ⅱ期比 ,其PA值差异皆有显著性意义 (t =2 .673 ,4.2 4 2 ,4.0 64 ,P <0 .0 1或 P <0 .0 0 1 ) ;Ⅰ期患者的E、A、E/A、S、D均与PA呈简单抛物线回归关系 (R =0 .70 5 ,0 .797,0 .852 ,0 .660和 0 .773 ,均P <0 .0 1 )。所有曲线都有一个极大点或极小点 ,将曲线明显地分为Ia和Ib两段。Ib段患者的左房内径明显大于Ia段 [(37± 3)mm对 (33± 3)mm ,P <0 .0 0 1 ]。结论 肺静脉反流速度的增大趋势可以间接地反映左室舒张功能的恶化趋势 ; Objective To explore the significance of the relationship between mitral inflow velocity patterns, pulmonary venous forward flow velocities and flow velocity of atrial reversal.Methods Mitral and pulmonary venous peak flow velocity patterns (including E, A, E/A ridio,S, D and PA) were recorded by the transthoracic Doppler echocardiography in 120 objects with different left ventricular (LV)diastolic functions, and then, the PA of different groups were compared, and the relationship between E,A,S,D,E/A ridio and PA were studied in patients whose LV diastolic disfunction were in Ⅰ stage.Results The PA was significantly different between Ⅰ stage LV diastolic disfunction and the normal, as well as between Ⅱ stage and Ⅰ stage or Ⅲ stage and Ⅱ stage (t= 2.673 , 4.242 , 4.064 , P< 0.01 or P< 0.001 ). Simple parabola regression analysis showed that the E, A, E/A, S and D were correlated with PA in Ⅰ stage (R= 0.705 , 0.797 , 0.852 , 0.660 , 0.773 , all P< 0.01 ), and the relationship between E/A radio and PA was most strong. Every curve had a maximal point or minimal point that divided obviously the curve into two parts: Ia and Ib. The left atrium diameter [(37±3)mm vs (33±3) mm, P< 0.001 ] in Ib part was larger than that in Ia part.Conclusions The increase trend of pulmonary venous reversal flow velocity can reflect indirectly the worsening of LV diastolic disfunction; It's essential that Ⅰ stage LV diastolic disfunction is divided into two levels: Ia and Ib.
