摘要
目的:探讨在高龄产妇宫颈性难产分娩中应用利多卡因联合间苯三酚的效果。方法:选取2022年3月—2023年3月茂名市妇幼保健院收治的宫颈性难产高龄产妇102例为观察对象,采用随机数字表法分为对照组与观察组,各51例。对照组给予地西泮治疗,观察组给予利多卡因联合间苯三酚治疗。比较两组宫口全开时间、总产程时间、产后出血量、产妇分娩方式及不良母婴结局。结果:观察组宫口全开时间、总产程时间短于对照组,差异有统计学意义(P<0.001);两组产妇产后出血量比较,差异无统计学意义(P>0.05)。两组顺产、剖宫产占比比较,差异无统计学意义(P>0.05);观察组钳产占比低于对照组,差异有统计学意义(P=0.025);观察组不良母婴结局发生率低于对照组,差异有统计学意义(P=0.029)。结论:在高龄产妇宫颈性难产分娩中,应用利多卡因联合间苯三酚可发挥协同作用,使宫颈软化,可缩短高龄产妇宫颈性难产的宫口全开时间、产程,减少不良母婴结局。
Objective:To explore the effect of lidocaine combined with phloroglucinol in the elderly women with cervical dystocia.Methods:A total of 102 elderly women with cervical dystocia admitted to Maoming Maternal and Child Health Hospital from March 2022 to March 2023 were selected as observation subjects.They were divided into control group and observation group according to random number table method,with 51 cases in each group.The control group was treated with diazepam,and the observation group was treated with lidocaine and phloroglucinol.The time of full cervical dilation,the total labor process duration,the amount of postpartum bleeding,the mode of delivery,and the adverse maternal and infant outcomes were compared between the two groups.Results:The time of full cervical dilation and the total labor process duration in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.001);there was no significant difference in the amount of postpartum bleeding between the two groups(P>0.05).There was no significant difference in the proportion of spontaneous delivery and cesarean section between the two groups(P>0.05);the proportion of forceps delivery in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.025);the total incidence of adverse maternal and infant outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.029).Conclusion:The application of lidocaine combined with phloroglucinol can exert synergistic effects in the elderly women with cervical dystocia,soften the cervix,shorten the time of full cervical dilation and labor process in the elderly women with cervical dystocia,and reduce adverse maternal and infant outcomes.
