窒息氧合技术的临床应用研究

摘要 窒息氧合是在无自主呼吸或机械通气的情况下进行的被动氧合方式。自发现半个多世纪以来,鉴于其在气道管理中延长无通气安全时限的潜力,尤其随着高流量鼻导管(HFNO)等设备的出现,该技术引起了广大研究者的兴趣。目前关于窒息氧合技术的研究主要包括在麻醉诱导、危重症护理及呼吸内科中的应用,还涵盖在困难气道、肥胖、产科和儿科等亚人群中的应用。然而,随着医学亚专科水平的进步,对麻醉管理技术也提出了更高的要求。本文对目前国内外学者对窒息氧合过程中气体交换机制的研究及其在临床各领域的应用,包括该技术在临床应用中的优劣势及改进措施进行综述,旨在为窒息氧合技术更加合理、安全地应用于临床加强证据基础,同时为临床麻醉医师提供新的研究思路。 Apneic oxygenation is passive oxygenation in the absence of spontaneous breathing or mechanical ventilation.Since its discovery more than half a century ago,the technique has attracted widespread interest due to its potential to extend the safe duration of non-ventilation in airway management,especially with the advent of devices such as high-flow nasal catheters.Current research on apneic oxygenation technology includes applications in anesthesia induction,critical care,and respiratory medicine,as well as in subpopulations with difficult airways,obesity,obstetrics,and pediatrics.However,with the development of medical subspecialty level such as surgery,higher requirements are put forward for anesthesia management techniques.This article reviews the current domestic and foreign scholars'research on the gas exchange mechanism in the process of asphyxial oxygenation and its application in various clinical fields,including the advantages and disadvantages of this technology in clinical application and improvement measures,aiming to strengthen the evidence base for the more reasonable and safe application of asphyxial oxygenation technology in clinical practice,and provide new research ideas for clinical anesthesiologists.
