摘要
目的探讨经皮脊柱内镜椎间孔入路治疗腰椎椎体后缘离断症(PARS)的临床疗效。方法回顾性分析2020年3月年至2021年3月枣庄矿业集团枣庄医院收治的80例PARS患者的临床资料,根据治疗方法不同分为对照组与观察组,每组40例。对照组行开放椎板间开窗手术治疗,观察组行经皮脊柱内镜椎间孔入路治疗。比较两组临床疗效、视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分。结果观察组治疗总有效率为97.50%,高于对照组的80.00%,差异有统计学意义(P<0.05)。术后,两组VAS评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。术后,两组ODI评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。两组均未发生明显并发症。结论采用经皮脊柱内镜椎间孔入路治疗PARS疗效确切,可减轻患者疼痛,促进腰椎功能恢复,无中转开放、神经根损伤、硬膜囊撕裂等并发症发生,可满足患者临床治疗需求,值得临床推广应用。
Objective To investigate the clinical effect of percutaneous spinal endoscopic foraminal approach in the treatment of posterior apoph-ysis ring separation(PARS)of lumbar vertebrae.Methods The clinical data of 80 patients with PARS of lumbar vertebra treated in Zaozhuang Min-ing Group Hospital from March 2020 to March 2021 were retrospectively analyzed,and they were divided into the control group and the observation group according to different treatment methods,with 40 cases in each group.The control group was treated with open fenestration,and the observa-tion group was treated with percutaneous spinal endoscopic foraminal approach.The clinical efficacy,visual analogue scale(VAS)score and Oswes-try dysfunction index(ODI)score were compared between the two groups.Results The total effective rate in the observation group was 97.50%,which was higher than 80.00%in the control group,the difference was statistically significant(P<0.05).After operation,the VAS scores of the two groups were lower than those before operation,and the observation group was lower than the control group,and the differences were statistically sig-nificant(P<0.05).After operation,the ODI scores of the two groups were lower than those before operation,and the ODI scores in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).There were no obvious complications in the two groups.Conclusion Percutaneous spinal endoscopic foraminal approach is effective in the treatment of lumbar PARS,which can reduce the pain of patients and promote the recovery of lumbar function without complications such as conversion opening,nerve root injury and dural sac tear,which can meet the clinical treatment needs of patients and is worthy of promotion and application.
