摘要
目的分析探讨红细胞参数在轻型地中海贫血(TT)和轻度缺铁性贫血(IDA)诊断中的应用价值。方法回顾性分析2021年6月至2023年5月江西医学高等专科学校第一附属医院收治的80例贫血患者病例资料,将其中40例轻型TT患者纳入A组,40例轻型IDA患者纳入B组,另外选取40名健康者纳入C组。比较3组患者红细胞参数[红细胞计数(RBC)、平均红细胞体积(MCV)、红细胞血红蛋白量(MCH)、平均红细胞血红蛋白浓度(MCHC)、红细胞体积分布宽度(RDW-CV)],采用受试者工作特征(ROC)曲线分析相关红细胞参数对于轻型TT患者与轻型IDA患者的鉴别诊断效能。结果A组RBC(3.6±0.5)×10^(12)/L、MCV(66±6)fl、MCH(24.2±2.1)pg、MCHC(319±14)g/L与B组RBC(2.9±0.4)×10^(12)/L、MCV(77±6)fl、MCH(23.6±2.1)pg、MCHC(296±13)g/L均低于C组RBC(4.6±0.5)×10^(12)/L、MCV(92±5)fl、MCH(30.5±1.9)pg、MCHC(343±9)g/L(P<0.05);A组RDW-CV(15.8±1.5)%与B组RDW-CV(18.8±1.9)%均高于C组RDW-CV(12.8±1.2)%(P<0.05);A组RBC、MCHC水平均高于B组(P<0.05);A组MCV、RDW-CV水平均低于B组(P<0.05);A组、B组MCH水平差异无统计学意义(P>0.05)。ROC曲线分析显示,RBC、MCV、MCHC、RDW-CV鉴别诊断轻型TT患者与轻型IDA患者的敏感度分别为77.5%、82.5%、92.5%、92.5%;特异度分别为87.5%、90.0%、75.0%、70.0%;AUC分别为0.862、0.910、0.891、0.881(P<0.05)。结论轻型TT患者与轻型IDA患者RBC、MCV、MCHC、RDW-CV水平存在差异,对于轻型TT与IDA的鉴别诊断具有较高效能,临床应用价值较高。
Objective To analyze and explore the application value of erythrocyte parameters in the differential diagnosis of mild thalassemia(TT)and mild iron deficiency anemia(IDA).Methods The case data of80 patients with anemia admitted to First Affiliated Hospital of Jiangxi Medical College from June 2021 to May2023 were retrospectively analyzed,40 of them with mild TT were included in group A,40 of them with mild IDA were enrolled as group B,and another 40 healthy subjects were selected as group C.The erythrocyte parameters[red blood cell count(RBC),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),mean corpuscular hemoglobin concentration(MCHC),red blood cell volume distribution width(RDW-CV)]were compared among the three groups.ROC curve was used to analyze the differential diagnostic efficiency of erythrocyte parameters for patients with mild TT and mild IDA.Results The levels of RBC,MCV,MCH and MCHC with(3.6±0.5)×10^(12)/L,(66±6)fl,(24.2±2.1)pg and(319±14)g/L in group A and(3.0±0.4)×10^(12)/L,(77±6)fl,(23.6±2.1)pg and(296±13)g/L in group B were lower than(4.6±0.5)×10^(12)/L,(92±5)fl,(30.5±1.9)pg and(343±9)g/L in group C(P<0.05)while the RDW-CV with(15.8±1.5)%in group A and(18.8±1.9)%in group B was higher than(12.8±1.2)%in group C(P<0.05).The RBC and MCHC were higher in group A than those in group B(P<0.05);while the MCV and RDW-CV were lower than those in group B(P<0.05).There was no significant difference in the levels of MCH between group A and B(P>0.05).ROC curve analysis showed that the sensitivities of RBC,MCV,MCHC and RDW-CV for the differential diagnosis of patients with mild TT and patients with mild IDA were 77.5%,82.5%,92.5%and 92.5%respectively;and the specificities were 87.5%,90.0%,75.0%and 70.0%;and the AUC were 0.862,0.910,0.891 and 0.881 respectively(P<0.05).Conclusion There were significant differences in RBC,MCV,MCHC and RDW-CV between patients with mild TT and patients with mild IDA,and the above indicators have high efficiency in the differential diagnosis between mild TT and IDA,which had a high clinical application value.
