摘要
目前国内外在孕期情志应激致胎损的机制研究及防治措施方面已经取得了较大的进展,但目前的研究还存在以下问题。第一,从理论上分析,孕期七情过极均可导致胎损,但目前除恐应激之外其他情志应激导致胎损的实验研究较少,需要进一步加强。第二,情志应激致胎损的防治方面,目前大部分研究关注的焦点在孕期情志的调理,对情志应激所致胎损的治疗探讨仍较少。第三,防治措施方面,多是采用现有适用于普通人群的食疗、五行音乐、运动、正念及心理干预方法,缺乏专门针对孕妇设计的方案。第四,各种干预疗法的效果缺乏大样本、多中心的高水平循证评估。今后应明晰单种情志或多种情志组合造成胎损的机制,并制定相应的防治措施。
At present,great progress has been made in the mechanism and prevention and treatment of fetal injury caused by emotional stress during pregnancy at home and abroad,but there are still the following problems.First,from the theoretical analysis,the seven emo-tions in pregnancy can lead to fetal injury,but at present,there are few experimental studies on fetal injury caused by emotional stress except for fear stress,which needs to be greatly strengthened.Second,in the prevention and treatment of fetal injury caused by emotional stress,most of the research focuses on the emotional conditioning during pregnancy,and there are still few discussions about the treat-ment of fetal injury caused by emotional stress.Third,in terms of prevention and treatment measures,most of the existing methods for the general population are adopted,such as diet,five-element music,exercise,mindfulness and psychological intervention,lack of special designs for pregnant women.Fourth,the effect of various intervention therapies lacks large sample,multi-center and high-level evidence-based evaluation.In the future,the mechanism of fetal injury caused by a single emotion or a combination of multiple emotions should be clarified,and corresponding prevention and treatment measures should be formulated.
