摘要
为了探讨影响经皮椎体成形术(percutaneous vertebroplasty,PVP)治疗骨质疏松性椎体压缩骨折(osteoporotic vertebral compression fractures,OVCF)术后24 h手术效果的影响因素,通过回顾性分析2015年1月—2022年1月邯郸市中心医院骨五科行PVP治疗的351名OVCF患者,根据Macnab量表将术后24 h手术效果分类,将患者分为满意组(A)和不满意组(B),分析患者骨折时间、术前VAS(visual analogue scale)评分、抑郁症病史等相关因素,将有统计学意义的变量进行二元Logistic回归分析。结果表明:两组比较,患者骨折时间、术前VAS评分、有无抑郁症病史均存在显著性差异(P<0.05)。Logistic回归分析显示骨折时间长、术前VAS评分低、有抑郁症病史为PVP术后24 h患者不满意的独立危险因素。可见PVP术后手术效果与患者骨折时间、术前VAS评分、有无抑郁症病史有关;骨折时间≤3周、术前VAS评分≥7分、无抑郁症的患者手术效果好,患者满意度更高。
To retrospectively analysis the influence of percutaneous vertebroplasty(PVP)in curing osteoporotic vertebral compression fractures(OVCF)within post-operative 24 h,351 patients with OVCF who underwent PVP treatment were from the fifth Orthopedic Department of Handan Central Hospital from January 2015 to January 2022.According to modified Macnab criteria in evaluating the surgery result of post-operative 24 h,the patients were divided into satisfied group(A)and unsatisfied group(B),counting the patient s fracture period,visual analogue scale(VAS)score before surgery,history of depression,comparing the datas between two groups,and Logistic regression analysis would be performed on the variables which were significantly statistical different.Comparing group A(n=56)with group B(n=56),the fracture period,VAS score before surgery,history of depression had significant statistics differences(P<0.05).Logistic regression analysis show that fracture period is short,VAS score before surgery was low,and retain the history of depression are independent risk factors for unsatisfied after PVP in 24 hours.The result within 24 post-surgery hours of PVP is related to patient s fracture period,preoperative VAS score,and history of depression.The patients with fracture time≤3 weeks,preoperative VAS score≥7,and history of depression would have excellent and good surgery results and higher satisfaction.
