右侧非返性喉返神经一例及相关诊疗探讨

A case of right non-recurrent laryngeal nerve and related diagnosis and treatment

摘要 非返性喉返神经是一种罕见的解剖变异,其从迷走神经分出,绕颈动脉后直接向颈内侧延伸,并进一步入喉,分出水平位置的高低决定了入喉走行的路径,因其走行路径多变,所以在甲状腺手术中有很高的损伤风险。本文回顾性分析非返性喉返神经1例,并复习相关文献资料;从术前检查、术中操作等多个诊疗环节,通过分析神经变异发生的解剖学基础、类型,探讨神经变异的临床评估方法,有助于加强变异神经的术中保护,降低损伤概率,从而提高术后患者的生活质量,减少临床医疗风险。 Non-recurrent laryngeal nerve is a rare anatomical variation.It is divided from the vagus nerve,extends directly to the medial side of the neck after around the carotid artery,and further entering the larynx.The level at which it separates from the vagus nerve determines the path into the larynx.Due to its variable route,there is a high risk of injury during thyroid surgery.In this paper,a case of non-recurrent laryngeal nerve was retrospectively analyzed,and the relevant literature was reviewed.From the preoperative examination,intraoperative operation and other diagnosis and treatment links,the clinical evaluation method of nerve variation is discussed by analyzing the anatomical basis and type of nerve variation,which is helpful to strengthen the intraoperative protection of variant nerves and reduce the probability of injury,so as to improve the quality of life of postoperative patients and reduce the clinical medical risk.
