3D Slicer技术在骨质疏松性骨折椎体强化术中的应用研究

摘要 目的探讨3D Slicer分析处理平台应用在骨质疏松性椎体压缩性骨折椎体强化术中的应用价值。方法选取2020年1月至2021年12月杭州市第九人民医院收治的择期行椎体强化术治疗的66例骨质疏松性椎体压缩性骨折患者作为研究对象,按随机数字表法将行3D Slicer软件辅助定位置钉的33例作为3D组和传统徒手定位置钉的33例作为对照组。观察并评估两组患者位置钉Gertzbein-Robbins分级、手术相关指标(单钉置入时间、手术时间、手术中出血量)、手术后指标(疼痛缓解的时间、手术后引流量、住院时间)和随访结果(对患者随访至术后1年,记录生存质量评分及并发症发生情况)。结果3D组患者Gertzbein-Robbins分级中A+B级的例数多于对照组A+B级的例数(P<0.05)。3D组患者单钉置入时间、手术时间、手术中的出血量、手术后疼痛缓解时间、手术后的引流量、住院时间均低于对照组患者(P<0.05)。两组随访时间中位数为(13.45±0.98)个月,3D组患者在各个随访时间点SF-12评分均高于对照组(P<0.05),随访截止时间3D组并发症发生率低于对照组(P<0.05)。结论3D Slicer分析处理平台应用在骨质疏松性椎体压缩性骨折椎体强化术中,可提高置钉的准确度,改善患者预后的症状且可降低术后并发症,患者生存质量高。 Objective To explore the application value of 3D Slicer analysis and processing platform in vertebral reinforcement surgery for osteoporotic vertebral compression fractures.Methods A total of 66 patients with osteoporotic vertebral compression fractures who underwent elective vertebral augmentation surgery at Hangzhou Ninth People’s Hospital from January 2020 to December 2021 were selected as the study subjects.They were randomly divided into a 3D group consisting of 33 patients who underwent 3D Slicer software assisted positioning and fixation,and a control group consisting of 33 patients who underwent traditional manual positioning and fixation.Observe and evaluate the Gertzbein-Robbins grading,surgical related indicators(single nail insertion time,surgical time,intraoperative bleeding),postoperative indicators(time of pain relief,operation drainage volume,hospital stay),and follow-up results(for patients followed up to 1 year after surgery,quality of life scores,record the incidence complications)in two groups of patients.Results The number of A group+B group cases in the Gertzbein-Robbins grading of patients in 3D group was higher than that in the control(P<0.05).The insertion time,surgical time,intraoperative bleeding volume,pain relief time,the volume of operation drainage,and the hospital time of the 3D group patients were lower than the control group(P<0.05).The median follow-up time for the two groups was(13.45±0.98)months.Each follow-up time point,the SF-12 score of 3D group was higher than the control group(P<0.05),the incidence complications of the 3D group was lower than that in the control group(P<0.05).Conclusion The application of 3D Slicer analysis and processing platform in vertebral reinforcement surgery for osteoporotic vertebral compression fractures can improve the accuracy of nail placement,improve clinical symptoms of patients,and reduce the incidence of surgical related complications.The quality of life of patients is high.
