摘要
全球大肠癌的发病率和病死率均位于前几位,并呈上升趋势,严重威胁人类的健康。西医主要采取以手术、放化疗为主的治疗方法,虽然能够解决部分癌肿,却对患者身心造成了创伤,中医药治疗充分发挥其整体论治、多靶点、不良反应小的特点,促进大肠癌患者的恢复。张红教授认为大肠癌的发生发展与脾、肝、肺、肾脏腑功能失调,正虚邪盛,气血津液代谢障碍,水湿、痰浊、热毒胶结于肠道,日久成积相关,其中脾虚证与大肠癌发生发展的全过程关系密切。辨证论治,将大肠癌分为脾气虚证、脾虚肝郁证、脾虚痰湿证、湿热蕴结证和脾肾两虚证型。张红教授在治法上提出,扶正应以健脾为主,兼顾疏肝、补肾;祛邪以祛湿化痰清热、解毒化瘀为主。扶正与祛邪并用,在减缓药物毒性、提高患者身体素质、改善临床症状及生活质量方面疗效颇佳。张红教授临床经验丰富,帮助众多患者实现了带瘤长期生存,因此,从脾论治大肠癌的思路值得推广,为丰富中医药治疗提供了一定的思路。
The incidence and mortality of colorectal cancer in the world are among the top few,and are on the rise,which seriously threatens human health.Western medicine mainly adopts surgery,radiotherapy and chemotherapy.Although it can solve some cancers,it has caused trauma to patients’body and mind.TCM medicine treatment gives full play to its characteristics ofT holistic treatment,multi-target and small side effects,and promotes the recovery of patients with colorectal cancer.Professor ZHANG Hong believes that the occurrence and development of colorectal cancer are related to the dysfunction of spleen,liver,lung and kidney.The excess of positive Qi(气)and evil,the metabolic disorder of Qi,blood and body fluid,the adhesion of water,dampness,phlegm and heat toxin to the intestine,and the accumulation over time,among which spleen deficiency syndrome is closely related to the whole process of the occurrence and development of colorectal cancer.According to syndrome differentiation and treatment,colorectal cancer can be divided into spleen-Qi deficiency syndrome,spleen deficiency and liver depression syndrome,spleen deficiency and phlegm-dampness syndrome,damp-heat accumulation syndrome and spleen-kidney deficiency syndrome.In the treatment,it is proposed that strengthening the body resistance should focus on strengthening the spleen,soothing the liver and tonifying the kidney.Eliminating pathogenic factors is mainly based on eliminating dampness,resolving phlegm,clearing heat,detoxifying and removing blood stasis.The combination of strengthening the body resistance and eliminating pathogenic factors has a good effect on slowing down drug toxicity,improving patients’physical fitness,improving clinical symptoms and quality of life.Professor ZHANG Hong’s rich clinical experience has helped many patients achieve long-term survival with tumors.Therefore,the idea of treating colorectal cancer from spleen is worth popularizing,which provides some ideas for enriching the treatment of TCM.
