摘要
目的:探究不同孕周血清及尿液尿酸测定对妊娠高血压疾病(HDP)发生及发展的预测作用。方法:回顾性分析中山市小榄人民医院2020年7月—2023年6月进行常规产检并诊断为HDP的孕早期孕妇(11~13周)106例的临床资料,将其分为妊娠高血压组(42例)、轻度子痫前期组(31例)、重度子痫前期组(33例),同期选取于我院接受常规产检的健康孕早期孕妇(11~13周)106例作为对照组,比较4组孕妇血清及尿液尿酸值及临床结局。结果:孕早、中、晚期,妊娠高血压组、轻度子痫前期组、重度子痫前期组血清尿酸水平高于对照组,重度子痫前期组血清尿酸水平高于轻度子痫前期组、妊娠高血压组,轻度子痫前期组血清尿酸水平高于妊娠高血压组,妊娠高血压组、轻度子痫前期组、重度子痫前期组血清尿酸水平随着孕周增长而升高,差异有统计学意义(P<0.05)。孕早、中、晚期,妊娠高血压组、轻度子痫前期组、重度子痫前期尿液尿酸水平均高于对照组,且重度子痫前期组高于轻度子痫前期组和妊娠高血压组,轻度子痫前期组高于妊娠高血压组,妊娠高血压组、轻度子痫前期组、重度子痫前期组血清尿酸水平随着孕周增长而升高,差异有统计学意义(P<0.05)。妊娠早、中、晚期血清及尿液尿酸水平对预测HDP的发生均具有一定价值(P<0.05),其中孕晚期血清及尿液尿酸水平在预测HDP发生时的曲线下面积最大,最佳截断值分别为385.56μmol/L、4866.81μmol/L。重度子痫前期组剖宫产、产后出血、胎儿窘迫、胎儿轻度窒息发生率均高于轻度子痫前期组和妊娠高血压组,差异有统计学意义(P<0.05)。结论:HDP孕妇血清及尿液尿酸水平随病情发展及孕周增长而不断上升,孕晚期血清及尿液尿酸水平预测HDP的价值最大,且HDP孕妇病情严重程度会影响孕妇围生结局。
Objective:To explore the predictive effect of serum and urine uric acid determination on the occurrence and develop-ment of pregnancy-induced hypertensive disease(HDP).Methods:A retrospective analysis was performed on one hundred and six pregnant women in the first trimester of pregnancy(11 to 13 weeks)diagnosed with HDP who underwent routine prenatal examina-tion in Zhongshan Xiaolan People's Hospital from July 2020 to June 2023,and they were divided into pregnancy hypertension group(42 cases),mild preeclampsia group(31 cases)and severe preeclampsia group(33 cases).In the same period,one hundred and six healthy pregnant women(11 to 13 weeks)who received routine birth examination in the hospital were selected as the con-trol group.The serum and urine uric acid values and clinical outcomes of pregnant women in 4 groups were compared.Results:In early,middle and late pregnancy,the serum uric acid level of the hypertensive group,the mild preeclampsia group and the severe preeclampsia group was higher than that of the control group,serum uric acid level in severe preeclampsia group was higher than that in mild preeclampsia group and pregnancy hypertension group,serum uric acid level in mild preeclampsia group was higher than that in pregnancy hypertension group,the serum uric acid level of pregnancy hypertension group,mild preeclampsia group and severe preeclampsia group increased with the increase of pregnancy week,and the difference was statistically significant(P<0.05);Urine uric acid levels in the early,middle and late trimesters of pregnancy hypertension,mild preeclampsia and severe preeclamp-sia were higher than those in the control group,the severe preeclampsia group was higher than the mild preeclampsia group and pregnancy hypertension group,and the mild preeclampsia group was higher than the pregnancy hypertension group;the serum uric acid level of pregnancy hypertension group,mild preeclampsia group and severe preeclampsia group increased with the in-crease of pregnancy week,and the difference was statistically significant(P<0.05);The serum and urine uric acid levels in early,middle and late pregnancy were of certain value in predicting the occurrence of HDP(P<0.05),the area under the curve of serum and urine uric acid level in the third trimester was the largest in predicting the occurrence of HDP,and the best cutoff values were 385.56μmol/L and 4,866.81μmol/L,respectively;The incidence of cesarean section,postpartum hemorrhage,fetal distress and mild fetal asphyxia in severe preeclampsia group was higher than that in mild preeclampsia group and pregnancy hypertension group,and the difference was statistically significant(P<0.05).Conclusion:Serum and urine uric acid levels in pregnant women with HDP increased with the progression of the disease and the growth of the gestational week.The value of serum and urine uric acid levels in late pregnancy in predicting HDP was the greatest,and the severity of the disease in pregnant women with HDP would affect the perinatal outcome of pregnant women.
