家庭Ⅱ期心脏康复结构化教育在冠脉支架植入术患者中的应用效果

摘要 目的探讨基于知信行理论的家庭Ⅱ期心脏康复结构化教育对冠脉支架植入术术后患者生活质量及再入院率的影响。方法将80例行冠脉支架植入术术后住院患者随机分为干预组和对照组,每组40例。对照组住院期间和出院后给予常规健康教育干预;干预组在常规健康教育的基础上实施家庭Ⅱ期心脏康复结构化教育干预。比较2组患者干预前和干预3个月后的生活质量[采用中国心血管病人生活质量评定问卷(CQQC)进行评价]及再入院率情况。结果干预组干预前后CQQC评分分别为(71.18±15.11)、(82.70±15.92)分,对照组分别为(71.85±15.64)、(70.13±15.64)分,2组干预前CQQC评分比较差异无统计学意义(P>0.05),干预组干预后CQQC评分显著高于对照组干预后(P<0.001);干预组再入院率显著低于对照组(5.0%比25.0%,P<0.05)。结论基于知信行理论的家庭Ⅱ期心脏康复结构化教育可有效改善冠脉支架植入术术后患者的体力状况和生活质量,降低患者的再入院率。 Objective Based on the theory of knowing,believing and acting,to explore the effect of family-based PhaseⅡcardiac rehabilitation with a structured education programme on the quality of lifeand readmission rate of patients after coronary stent implantation.Methods Eighty hospitalized patients after coronary stent implantation were randomly divided into the intervention group and the control group,with 40 patients in each group.The control group was given routine health education interventions during hospitalization and after discharge;the intervention group was further given a structured education programme for family-based PhaseⅡcardiac rehabilitation on the basis of routine health education.The quality of life,evaluated by the Chinese Cardiovascular Quality of Life Rating Question naire for Cardiovascular Patients(CQQC),and the readmission rate of patients in the 2 groups were compared before and 3 months after the intervention.Results The CQQC scores before and after intervention were(71.18±15.11)and(82.70±15.92)in the intervention group and(71.85±15.64)and(70.13±15.64)in the control group,respectively;the difference between the CQQC scores of the 2 groups before the intervention was not statistically significant(P>0.05);the CQQC scores of the intervention group were significantly higher than those of the control group after intervention(P<0.001);the readmission rate of the intervention group was significantly lower than that of the control group(5.0%vs 25.0%,P<0.05).Conclusion Based on the theory of knowing,believing,and doing,the structured education programme for family-based PhaseⅡcardiac rehabilitation could effectively improve physical conditions and quality of life for patients after coronary stenting and reduce the readmission rate of patients.
