摘要
目的 探讨健阳益坎方联合左卡尼汀口服液治疗肥胖型少弱精子症患者的疗效及对其精子质量及性激素[卵泡刺激素(Follicle stimulating hormone, FSH)、黄体生成素(Luteinizing hormone, LH)、雌二醇(Estradiol, E_(2))、睾酮(Testosterone, T)]的影响。方法 选取2020年1月—2022年10月期间宜兴市人民医院生殖中心和泌尿外科、张家港市中医医院男科、陕西省中医医院男科门诊收治的肥胖型少弱精子症患者90例作为研究对象,按简单随机数字表法分为对照组和研究组,每组各45例。对照组采取左卡尼汀口服液,研究组在对照组基础上采取健阳益坎方,均治疗3个月。观察比较两组患者临床疗效、不良反应发生情况及治疗前后中医证候积分、精子质量、性激素指标水平。结果 (1)临床疗效:治疗后研究组临床总有效率95.56%(43/45)明显高于对照组82.22%(37/45),差异有统计学意义(P<0.05)。(2)中医证候积分:治疗后两组患者主症与次症分值均较治疗前降低,差异有统计学意义(P<0.05);且研究组主症与次症分值均明显低于对照组,差异有统计学意义(P<0.05)。(3)精子质量:治疗后两组患者精子浓度、精液量、前向运动百分率、精子总数均较治疗前升高,差异有统计学意义(P<0.05);且研究组精子浓度、精液量、前向运动百分率、精子总数均明显高于对照组,差异有统计学意义(P<0.05)。(4)性激素:治疗后两组患者血FSH、LH、T水平均较治疗前升高,E_(2)水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组血FSH、LH、T水平均明显高于对照组,E_(2)水平明显低于对照组,差异有统计学意义(P<0.05)。(5)安全性:治疗期间,研究组不良反应发生率4.44%(2/45)与对照组2.22%(1/45)比较,差异无统计学意义(P>0.05)。结论 联合采取健阳益坎方及左卡尼汀口服液治疗肥胖型少弱精子症患者,可缓解临床症状,调节性激素水平,改善精子质量,利于提升疾病整体干预效果,且具有安全性。
Objective To explore the effectiveness of Jianyang Yikan Prescription combined with Levocarnitine oral liquid in the treatment of obese oligoasthenospermia patients and the impact on sperm quality and sex hormones[follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2)),and testosterone(T)].Methods Ninety obese oligoasthenospermia patients admitted to the Reproductive Center and Department of Urology of Yixing People′s Hospital,the Department of Andrology of Zhangjiagang Hospital of Traditional Chinese Medicine,and the Andrology Outpatient Clinic of Shaanxi Provincial Hospital of Chinese Medicine from January 2020 to October 2022 were selected.They were divided into a control group and a study group by random number table,with 45 in each group.The control group received Levocarnitine oral liquid,while the study group was treated with Jianyang Yikan Prescription on the basis of the control group.The treatment lasted for 3 months.Clinical efficacy,adverse reactions,traditional Chinese medicine(TCM)syndrome scores,sperm quality,and sex hormones in the two groups were observed and compared before and after treatment.Results Clinical efficacy:The total effective rate in the study group(95.56%,43/45)was higher than that in the control group(82.22%,37/45)(P<0.05).TCM syndrome scores:Scores for primary and secondary symptoms decreased in both groups after treatment(P<0.05),and the study group showed lower scores than the control group(P<0.05).Sperm quality:Sperm concentration,semen volume,percentage of progressive motility,and total sperm count increased in both groups after treatment(P<0.05),and these parameters were better in the study group compared with the control group(P<0.05).Sex hormones:FSH,LH,and T were elevated,while E_(2) decreased in both groups after treatment(P<0.05),and the study group had more significant improvements than the control group(P<0.05).Safety:The incidence of adverse reactions was not significantly different between the study group(4.44%,2/45)and the control group(2.22%,1/45)(P>0.05).Conclusion Jianyang Yikan Prescription combined with Levocarnitine oral liquid could alleviate clinical symptoms,regulate sex hormones,and improve sperm quality in the treatment of obese oligoasthenospermia patients,thus enhancing the overall effectiveness of disease intervention,with high safety.
