摘要
目的探讨自拟排石汤联合一次性电子输尿管肾盂镜Ⅰ期治疗上尿路结石的疗效。方法选取就诊于重庆市巴南区中医院泌尿外科住院患者60例。采用奇偶数字法分为治疗组和观察组,各30例。2组患者均采用一次性电子输尿管肾盂镜手术治疗,并留置输尿管支架以及术后常规治疗。治疗组在观察组基础上持续口服中药2周,观察2组治疗前后中医证候评分、血常规及尿常规白细胞指数以及泌尿系彩超结石大小数值。结果2组均能有效清除上尿路结石,但对于清除体内残留结石治疗,治疗组优于观察组,差异具有统计学意义(P<0.05)。术后,2组临床症状消失率比较,差异具有统计学意义(P<0.05);术后8周,2组清除结石显效率及有效率比较差异具有统计学意义(P<0.05)。结论中药汤剂联合一次性输尿管电子肾盂内窥镜针对输尿管上段结石治疗效果可靠,能够有效清除术后残余结石,降低肾绞痛发生率,因此值得推广。
Objective To significantly improve the efficiency of self-made Paishi decoction combined with disposable electronic ureteropelvoscope in phase I in the treatment of upper urinary tract stones.Methods Sixty patients in Ba’nan District Hospital of Traditional Chinese Medicine were selected,with 30 cases in each.They were divided into treatment group and observation group by odd-even number method.All patients in 2 groups were treated with disposable electronic ureteropyeloscopy,ureteral stent indignant and postoperative routine treatment.The treatment group continued to take Chinese medicine orally for 2 weeks on the basis of the observation group,and the TCM syndrome score,blood routine and urine routine white blood cell index and color ultrasound stone size of urinary system before and after treatment were observed.Results Both groups could effectively remove upper urinary tract stones,but the treatment group was better than the observation group for the removal of residual stones,and the difference was statistically significant(P<0.05).After operation,there was a statistically significant difference in the disappearance rate of clinical symptoms between the two groups(P<0.05).At 8 weeks after operation,there were statistically significant differences in the significant efficiency and effective rate of stone removal between the two groups(P<0.05).Conclusion The therapeutic effect of Chinese medicine decoction combined with disposable electronic ureteropelvic endoscope for upper ureteral calculi is reliable.It can effectively remove residual stones after surgery and reduce the incidence of renal colic,so it is worth promoting.
