摘要
目的 比较掌侧锁定接骨板与外固定支架治疗桡骨远端C2/C3型骨折患者的效果。方法 选取2019年1月-2022年4月收治的桡骨远端C2/C3型骨折72例,以随机数字表法分成观察组(36例)与对照组(36例),对照组给予外固定支架治疗,观察组给予掌侧锁定接骨板治疗,比较两组围术期指标、影像学指标、腕关节活动度、握力、腕关节功能及并发症。结果 观察组手术时间、住院时间、骨折愈合时间较对照组更长(P<0.05),术中出血量更多(P<0.05);两组术后桡骨高度、掌倾角、尺偏角增大(P<0.05),观察组较对照组更大(P<0.05);两组术后腕关节背伸、旋前、旋后活动度增大(P<0.05),观察组较对照组更大(P<0.05);两组术后握力提高(P<0.05),观察组较对照组更高(P<0.05);两组术后腕关节患者自行评估量表(PRWE)评分降低(P<0.05),观察组较对照组更低(P<0.05);观察组(13.89%)并发症发生率与对照组(8.33%)比较,差异无统计学意义(P>0.05)。结论 与外固定支架相比,桡骨远端C2/C3型骨折患者采用掌侧锁定接骨板治疗,能够改善影像学指标、腕关节活动度,提高患者握力及腕关节功能,但该术式创伤大、恢复慢,临床上可根据具体情况进行选择。
Objective To compare the effectiveness of treating C2/C3 type distal radius fractures with palmar locking plate and external fixation bracket.Methods A total of 72 patients with C2/C3 distal radius fractures who were admitted in our hospital from January 2019 to April 2022 were selected,and divided into the study group(36 cases)and the control group(36 cases)by random number table method.The control group was received external fixation with a stent,while the study group was received treatment with a palmar locking plate.The perioperative and imaging indicators,wrist joint mobility,grip strength,wrist joint function,and complications of two groups were compared.Results In comparision of the control group,the surgery time,hospitalization time,and fracture healing time of the study group were longer(P<0.05),and the intraoperative blood output was more(P<0.05).After surgery,the radius height,palmar inclination angle,and ulnar deviation angle in both groups were increased(P<0.05),and in comparision of the control group,the study group was larger(P<0.05).After surgery,the wrist joint dorsiflexion,pronation,and supination range of motion in both groups were increased(P<0.05),and in comparision of the control group,the study group was greater(P<0.05).After surgery,the grip strength in both groups were increased(P<0.05),and in comparision of the control group,the study group was higher(P<0.05).After surgery,the PRWE scores in both groups were decreased(P<0.05),and in comparision of the control group,the study group was lower(P<0.05).In the incidence of complications,there was no statistically significant difference between the study group(13.89%)and the control group(8.33%)(P>0.05).Conclusion Compared with external fixation brackets, the treatment of C2/C3 type distal radius fractures with palmar locking plates can improve imaging indicators, wrist joint mobility, grip strength, and wrist joint function. However, this surgical method has significant trauma and slow recovery.
