摘要
目的研究富血小板纤维蛋白(PRF)在骨再生中的作用。方法选取24只新西兰大白兔,随机分为正常组、模型组、研究组及材料对照组,每组6只,制作兔腓骨骨缺损模型。正常组不做处理;模型组只造模;研究组将PRF和明胶海绵植入模型中;材料对照组只植入明胶海绵。6周后观察骨缺损处大体形态并进行X线摄片、HE染色及免疫组化,对结果进行统计学分析。结果大体观察X线摄片示正常组无异常;模型组有少量骨痂形成;研究组有规整骨痂形成;材料对照组有不规则骨痂形成。HE染色示正常组骨小梁完整;模型组未见明显骨小梁再生;研究组骨小梁再生明显;材料对照组部分骨小梁再生。ELISA结果示研究组、材料对照组较模型组骨形态发生蛋白2(BMP-2)显著升高,差异有统计学意义(P<0.05);血管内皮生长因子(VEGF)升高,差异无统计学意义(P>0.05);研究组较材料对照组BMP-2显著升高,差异具有统计学意义(P<0.05),VEGF升高但差异无统计学意义(P>0.05)。CD34结果示研究组和材料对照组CD34含量显著高于正常组和模型组,研究组和材料对照组之间差异有统计学意义(P<0.05)。结论PRF通过升高BMP-2和VEGF的表达,增加骨缺损处的血管及新骨的生成来促进骨缺损的修复。
Objective To investigate the role of platelet-rich fibrin in bone regeneration.Methods Twenty-four New Zealand white rabbits were randomly divided into normal group,model group,study group and material control group to make a rabbit fibula bone defect model.The normal group was not treated;the model group only makes models;the study group implanted PRF-gelatin sponge into the model;the material control group implanted gelatin sponge.6 weeks later,the gross morphology of the bone defect was observed and X-ray taken,HE staining and immunohistochemistry.The results were analyzed for statistical analysis.Results The general observation of X-ray film showed no abnormality in the normal group;a few callus were formed in the model group;the study group had regular bone callus formation,and the material control group had irregular bone callus formation;HE staining showed intact trabecular bone in the normal group;no significant trabecular regeneration in the model group;the trabecular bone regeneration was evident in the study group;some trabecular bone regeneration in the material control group;The elisa results showed that the bone morphogenic protein 2(BMP-2)was significantly increased in the study group and the material control group compared with the model group,which showed statistical significance(P<0.05);vascular endothelial growth factor(VEGF)was increased but not significant(P>0.05);the bone morphogenic protein 2(BMP-2)was significantly increased in the study group compared with the material control group,and the difference was statistically significant(P<0.05),while VEGF was increased but not statistically significant(P>0.05).The CD34 results showed that the CD34 content of the study group and the material control group was significantly higher than that of the normal group and the model group,and there were statistically significant differences between the study group and the material control group(P<0.05).Conclusion The PRF promotes the repair of bone defects by increasing the expression of BMP-2 and VEGF to increasing the blood vessels and new bone production at the bone defects.
