摘要
目的 探究视神经磁共振高清弥散张量成像技术在甲状腺相关性眼病活动性分期中的应用价值。方法 选择2020年6月至2023年6月在阳江光明眼科医院接受治疗的甲状腺相关性眼病患者60例作为研究对象,采用临床活动性评分系统对患者的活动性分期分为活动期(26例)和非活动期(34例)。60例患者均接受眼眶常规MRI序列扫描、视神经磁共振高清弥散张量成像技术序列扫描,对双侧视神经的各向异性分数、表观弥散系数、轴向弥散率、径向弥散率进行测算,并将活动期与非活动期患者的各项测算结果进行比较,同时分析各定量参数值与临床活动性分期的相关性。结果 甲状腺相关性眼病活动性分期患者的各向异性分数小于甲状腺相关性眼病非活动性分期患者,差异有统计学意义(P<0.05);甲状腺相关性眼病活动性分期患者和甲状腺相关性眼病非活动性分期患者表观弥散系数、轴向弥散率、径向弥散率上对比差异无统计学意义(P>0.05)。活动性眼的视神经临床活动性评分是(3.94±0.22)分,非活动性眼的视神经临床活动性评分是(1.33±0.14)分,各向异性分数与临床活动性评分呈负相关,临床活动性评分与表观弥散系数、轴向弥散率、径向弥散率之间不存在相关性(P>0.05)。结论 视神经磁共振高清弥散张量成像技术对于甲状腺相关性眼病患者的相关数据可为甲状腺相关性眼病活动性分期辅以可靠客观依据。
Objective To explore the application value of high-definition diffusion tensor imaging technique of optic nerve magnetic resonance in active staging of thyroid-associated ocular diseases.Methods A total of 60 patients with thyroid-related eye disease treated in Yangjiang Guangming Eye Hospital from June 2020 to June 2023 were selected as the main subjects of this study.The clinical activity scoring system was used to evaluate the stages of the patients′activity,and according to the evaluation results,they were divided into the active stage(n=26)and the inactive stage(n=34).All sixty patients underwent orbital routine MRI serial scanning and optic nerve magnetic resonance high-resolution diffusion tensor imaging serial scanning.The anisotropy fraction,apparent diffusion coefficient,axial diffusion rate,and radial diffusion rate of the bilateral optic nerve were measured,and the measurement results of active and inactive patients were compared and analyzed.At the same time,the correlation between the quantitative parameters and the stage of clinical activity was analyzed.Results The anisotropy score of patients with the active stage of thyroid-associated eye disease was lower than that of patients with the inactive stage of thyroid-associated eye disease,which was statistically significant(P<0.05).There was no statistical significance in diffusion coefficient,axial diffusion rate,and radial diffusion rate between patients with active stage of thyroid-associated eye disease and patients with non-active stage of thyroid-associated eye disease(P>0.05).The clinical activity score of the optic nerve in active eyes was(3.94±0.22),and that in inactive eyes was(1.33±0.14).The anisotropy score was negatively correlated with the clinical activity score.There was no correlation between clinical activity score and performance diffusion coefficient,axial diffusion rate,and radial diffusion rate(P>0.05).Conclusion The high-resolution diffusion tensor imaging technique of optic nerve magnetic resonance can provide reliable objective evidence for the active staging of thyroid-associated ophthalmopathy.
