从“湿热致瘀酿毒”辨治幽门螺杆菌胃炎炎癌转化 被引量:1

摘要 幽门螺杆菌(Hp)感染为慢性胃炎的主要病因,可加速慢性胃炎向胃癌转化的进程,为胃癌癌前病变的危险因素。吕文亮教授认为Hp属外感湿热毒邪,与内生湿热相合,潜藏中焦,日久化瘀酿毒,损伤胃络。其中瘀毒是胃炎向胃癌转化的关键转折点。针对湿热久伏,致瘀酿毒的病机特点,以祛邪、透邪、扶正为治疗原则,治疗上把握各阶段病机特点,早期治以清热化湿,中后期重视配合活血通络,祛瘀解毒,同时兼顾扶正,清养胃阴以助透达瘀热. Helicobacter pylori infection is the main cause of chronic gastritis,which can accelerate the transformation of chronic gastritis into gastric cancer,and is a risk factor for precancerous lesions of gastric cancer.Professor LYU Wenliang believes that Helicobacter pylori is an exogenous pathogenic factor of damp,heat and toxin,which is combined with endogenous humid heat,hidden in the middle Jiao.During prolonged course,dampness and heat transform into toxicity and blood stasis,which damages the stomach collaterals.And it is the key turning point in the transformation of gastritis to gastric cancer.In view of the pathogenesis characteristics of damp and heat,causing stasis and poisoning,professor LYU regards eliminating pathogenic factors,clearing evil and strengthening the body as the treatment principle.He grasps the pathogenesis characteristics of each stage in treatment.In the early stage,clearing heat and dampness should be used.And in the middle and late stage,attention should be paid to cooperating with activating blood circulation and removing blood stasis and detoxification.At the same time,both promoting normal and nourishing yin should be taken into account to help dispel stasis and heat.
