摘要
目的:基于团队资源管理(Team Resource Management,TRM)构建三级医院用药安全管理方案,并评价其应用效果,为改善患者用药安全提供参考。方法:从团队资源管理的领导统筹、状况监测、互助合作及有效沟通4个模块构建三级医院用药安全管理方案,并制定具体实施策略。比较实施前后的用药关键过程指标、护士用药安全能力及安全态度。结果:实施后用药类接近差错事件例数增加,药品配置2小时内执行率、用药前双人核对匹配率、静脉类药品滴速记录达标率均有改善(P<0.05),护士的用药安全能力得分、安全态度得分均高于实施前(P<0.05)。结论:基于TRM的用药安全管理方案可提升护士的安全用药能力,规范用药执行过程,同时能够提升医务人员的团队安全文化意识。
Objective:To construct a medication safety management plan for tertiary hospitals based on Team Resource Management(TRM)and evaluate its application effectiveness,and to provide a reference for improving the medication safety of patients.Methods:We constructed a tertiary hospital medication safety management plan from 4 modules of TRM:leadership coordination,condition monitoring,mutual assistance and cooperation,and effective communication,and developed specific implementation strategies as well.Key process indicators of medication,nurse medication safety ability,and safety attitude were compared before and after the implementation of plan.Results:After the implementation of plan,the number of medication near error events increased,drug configuration execution rate within 2 hours,pre-medication double check matching rate,and intravenous drug infusion velocity recording compliance rate all improved(P<0.05).The scores of nurse medication safety ability and safety attitude were higher than that before the implementation of plan(P<0.05).Conclusion:The medication safety management plan based on TRM can enhance the safety medication ability of nurses,standardize the medication execution process,and enhance the safety culture awareness of medical staff teams.
