摘要
目的探讨中药灌肠联合多西环素、甲硝唑对合并慢性子宫内膜炎(CE)的不孕患者治疗效果及辅助生育结局影响。方法选取2021年6月至2022年5月广东省妇幼保健院收治的80例合并CE的不孕患者,采用随机数字表法将其分为治疗组和对照组,每组各40例。对照组采用多西环素、甲硝唑治疗,治疗组在此基础上加用中药灌肠治疗。比较两组患者治疗前后的疗效。结果两组患者的临床总有效率、子宫内膜形态疗效、妊娠结局、气虚乏力积分比较,差异无统计学意义(P>0.05)。治疗组治疗后的下腹坠痛积分、月经不调积分、白带异常积分、面色萎黄积分、脉沉细积分、血管阻力指数(RI)和血流搏动指数(PI)均低于对照组,差异有统计学意义(P<0.05)。另外,治疗组治疗后的内膜厚度、血流指数(FI)、子宫内膜病理组织疗效均高于对照组,差异有统计学意义(P<0.05)。结论CE合并不孕患者采取中药灌肠联合多西环素、甲硝唑治疗可有效提升临床疗效,改善中医证候,提高辅助生育成功率。
Objective To explore the therapeutic effect and assisted reproductive outcomes of traditional Chinese medicine enema combined with Doxycycline and Metronidazole on infertility patients with chronic endometritis(CE).Methods A total of 80 infertility patients with CE in Guangdong Provincial Maternity and Child Health Hospital from June 2021 to May 2022 were selected and divided into treatment group and control group by random number table method,with 40 cases in each group.The control group was treated with doxycycline and metronidazole,and the treatment group was treated with Chinese medicine enema on this basis.The curative effect before and after treatment were compared between the two groups.Results There were no statistically significant differences in the total clinical effective rate,endometrial morphology efficacy,pregnancy outcome and Qi deficiency and fatigue score between the two groups(P>0.05).After treatment,the lower abdominal pain score,scores of menstrual disorders,abnormal vaginal discharge,pale complexion,pulse depth,vascular resistance index(RI),and pulsatile index(PI)in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).In addition,the treatment group showed higher endometrial thickness,blood flow index(FI),and endometrial pathological tissue efficacy than the control group after treatment,with statistically significant differences(P<0.05).Conclusion Traditional Chinese medicine enema combined with Doxycycline and Metronidazole can effectively improve clinical efficacy,improve traditional Chinese medicine syndromes,and increase the success rate of assisted reproduction in patients with infertility caused by CE.
