摘要
为了提高小儿急性阑尾炎超声报告阳性率和超声诊断符合率。收集2019年1月—2021年12月手术确诊为小儿急性阑尾炎患者89例,采用Fisher精确概率法及χ2检验比较不同年资医师超声首诊时临床资料、超声检查时报告的文字描述质量和图像质量、超声报告阳性率、超声诊断符合率及超声诊断类型的差异性,质控管理组对不同年资医师对超声诊断小儿急性阑尾炎的认知进行调查。结果发现不同年资医师在超声检查时首诊的临床资料、超声报告的文字描述质量、图像质量方面差异有统计学意义(P<0.05);不同年资医师超声报告阳性率差异无统计学意义(P>0.05);不同年资医师在超声诊断符合率、诊断类型方面比较差异有统计学意义(P<0.05);15%医生对部分类型的小儿急性阑尾炎认知不全,30%医生认为可以给出结论型诊断,32.5%医生认为可以诊断,但为了避免误诊不愿给出结论型诊断。小儿急性阑尾炎超声报告阳性率及超声诊断符合率均有待提高,主要原因为医师对该病认识不足及部分医师为避免误诊不愿出具结论型报告。
To improve the rate of positive ultrasound reports and ultrasound diagnostic compliance in pediatric acute appendicitis,89 patients with surgically confirmed diagnosis of pediatric acute appendicitis were collected from January 2019 to December 2021,and Fisher′s exact probability method andχ2 test were used to compare the clinical data of the first ultrasound consultation of physicians of different seniority,the quality of text description and image quality of report of ultrasound examination,the positive rate of ultrasound report,the compliance rate of ultrasound diagnosis,and the differences in the type of ultrasound diagnosis.The quality control management team investigated the knowledge of physicians of different seniority about ultrasound diagnosis of pediatric acute appendicitis.The results found statistically significant differences between physicians of different seniority in terms of clinical data of the first visit at ultrasound examination,quality of textual description of ultrasound report,and quality of images(P<0.05);the difference in positive rate of ultrasound report between physicians of different seniority was not statistically significant(P>0.05);and the difference in comparison of ultrasound diagnostic compliance rate and diagnostic type between physicians of different seniority was statistically significant(P<0.05);15%of physicians had incomplete knowledge of some types of pediatric acute appendicitis,30%held they could give a conclusive diagnosis,and 32.5%held they could diagnose it but they were reluctant to give a conclusive diagnosis to avoid misdiagnosis.The positive rate of ultrasound reports and the rate of compliance with ultrasound diagnosis of pediatric acute appendicitis need to be improved,mainly due to the lack of the physician awareness of this disease and the reluctance of some physicians to issue conclusive reports in order to avoid misdiagnosis.
