摘要
目的探究急性心力衰竭心衰,伴肾功能不全患者实施冻干重组人脑利钠肽与沙库巴曲缬沙坦钠结合治疗的效果。方法随机选取2021年4月-2023年4月淄博市市立医院就诊的70例急性心衰伴肾功能不全患者为研究对象,采用随机数表法将其分为两组,每组35例。两组患者均实施常规治疗,对照组加入沙库巴曲缬沙坦钠治疗,观察组在对照组基础上加以冻干重组人脑利钠肽治疗。比较两组治疗后的肾功能与心功能状况,并观察两组治疗期间存在的不良反应发生情况。结果治疗后,观察组尿素氮、肌酐、B型钠尿肽指标均显著低于对照组,差异有统计学意义(P均<0.05);观察组左室射血分数、血流流速比值水平高于对照组,左心室重塑指数与左心房容积指数水平低于对照组,差异有统计学意义(P均<0.05);观察组的不良反应发生率为11.43%,低于对照组的34.29%,差异有统计学意义(χ^(2)=5.185,P<0.05)。结论对急性心衰伴肾功能不全患者实施冻干重组人脑利钠肽与沙库巴曲缬沙坦钠结合治疗具有较为积极的临床疗效,能够显著改善患者的肾功能与心功能情况,减少不良反应的发生,对改善患者生活质量与整体预后有积极影响。
Objective To explore the effect of freeze-dried recombinant human brain natriuretic peptide combined with sacubactril valsartan sodium in the treatment of acute heart failure with renal insufficiency.Methods A total of 70 patients with acute heart failure with renal insufficiency who were treated in Zibo Municipal Hospital from April 2021 to April 2023 were selected randomly as the study objects,and were divided into two groups by random number table method,with 35 patients in each group.Patients in both groups received conventional treatment,the control group was treated with sacubactril valsartan sodium,and the observation group was treated with freeze-dried recombinant human brain natriuretic peptide on the basis of the control group.The renal function and cardiac function of the two groups after treatment were compared,and the occurrence of adverse reactions between the two groups during treatment was observed.Results After treatment,the indexes of urea nitrogen,creatinine and B-type natriuretic peptide in observation group were significantly lower than those in control group,and the differences were statistically significant(all P<0.05).The ratios of left ventricular ejection fraction and blood flow velocity in observation group were higher than those in control group,and the levels of left ventricular remodeling index and left atrial volume index were lower than those in control group,and the differences were statistically significant(all P<0.05).The incidence of adverse reactions in the observation group was 11.43%,lower than 34.29%in the control group,and the difference was statistically significant(χ^(2)=5.185,P<0.05).Conclusion The combination of freeze-dried recombinant human brain natriuretic pep-tide and sackubactril valsartan sodium in the treatment of acute heart failure with renal insufficiency has relatively positive clinical efficacy,which can significantly improve the renal function and cardiac function of patients,reduce the occurrence of adverse reactions,and have a positive impact on improving the quality of life and overall prognosis of patients.
