摘要
膜性肾病病因未明者为原发性膜性肾病(PMN),可归属于“水肿病”范畴。“阳化气,阴成形”理论源于《素问·阴阳应象大论篇》,是对阴阳特性的高度概括。“阳化气”与“阴成形”代表着阴阳的两种运动形式,由精血津液转化为能量,需依靠阳的气化作用;由气化生精血津液,离不开阴的成形作用。二者既互为能量来源、又相互补充,维持人体生理功能。阳化气不足、阴成形太过,造成痰饮、水湿、瘀血等有形之物累积,从而产生阳虚癥积的病理状态,是PMN的关键病机,温阳化积是主要治法。结合临床病案对基于“阳化气,阴成形”应用温阳化积法治疗原发性膜性肾病进行介绍。
Membranous nephropathy with unknown causes is classified as primary membranous nephropathy(PMN),falling within the category of shuizhong(edema)in TCM.The theory of“yang transforms into qi,while yin constitutes form”originates from the Su Wen-Yin Yang Ying Xiang Da Lun Pian(Plain Questions:Comprehensive Discourse on Phenomena Corresponding to Yin and Yang)of Huangdi Neijing,providing a highly summarized understanding of the characteristics of yin and yang.“Yang transforms into qi”and“yin constitutes form”represent the two forms of movement for yin and yang.Essence,blood,and body fluids are turned into energy through the transformation into qi by yang,while the generation of essence,blood,and body fluids through qi transformation is inseparable from the form-constituting role of yin.Both aspects not only serve as mutual energy sources but also complement each other,maintaining human physiological functions.Insufficiency in the yang aspect and excessive activity of yin result in the accumulation of tangible substances such as phlegm,fluid,water dampness,and blood stasis.This leads to the pathological state of yang deficiency syndrome,which is the key pathogenesis of PMN.The method of warming yang and transforming accumulations is the primary treatment approach.This paper introduces the clinical application of this method in treating PMN based on the theory of“yang transforms into qi,while yin constitutes form”,with clinical cases.
