时间飞跃磁共振血管造影预测颅外颈内动脉狭窄研究

摘要 目的:评估颅内时间飞跃核磁共振血管造影(TOF-MRA)对预测颅外颈内动脉狭窄(ICAS)的诊断价值。方法:回顾性分析2020年8月至2021年12月在河北中石油中心医院就诊并诊断为中度(50%~69%)或重度(70%~99%)单侧颅外ICAS的51例患者病例资料(病变组),同时选取无颅外ICAS的患者100例为对照组。在TOF-MRA源图像上测量两组双侧颈内动脉的腔内信号强度(SI),比较两组对侧与病变侧的SI比值(SIR)。结果:病变组颅内右侧C4段SIR值1.463显著高于对照组的1.035,两组比较差异具有统计学意义(t=7.209,P<0.001)。病变组左侧狭窄的平均SIR为1.362,对照组平均SIR为1.000。右侧C4狭窄的受试者工作特征(ROC)曲线的临界值为1.086,灵敏度、特异度分别为75%和81%;ROC曲线下面积(AUC)为0.81;左侧C4狭窄的ROC曲线的临界值为1.104,灵敏度、特异度分别为70%和84%;AUC为0.80。结论:TOFMRA上的SIR可以用于预测颅外ICAS的发生。 Objective:To assess the diagnostic value of intracranial time of flight(TOF)magnetic resonance angiography(MRA)in predicting extracranial internal carotid artery stenosis(ICAS).Methods:A retrospective analysis was conducted on 51 patients(lesion group)were diagnosed as moderate(from 50%to 69%)or severe(from 70%to 99%)unilateral extracranial ICAS at Hebei Petro China Central Hospital from August 2020 to December 2021.At the same time,100 patients without extracranial ICAS were selected as the control group.The intracavitary signal intensity(SI)on the images,which originated from TOF-MRA,of bilateral internal carotid arteries of two groups were measured.And then,the SI ratio values of opposite side to diseased side between two groups were compared.Results:The SIR value(1.463)of the right intracranial C4 segment in the lesion group was significantly higher than that(1.035)of the control group,and the difference between the two groups was statistically significant(t=7.209,P<0.001).The average SIR value of left stenosis in the lesion group was 1.362,while that of the control group was 1.000.The critical value of the operating characteristic curve(ROC)of subjects with right C4 stenosis was 1.086,and the sensitivity and specificity of that were respectively 75%and 81%.The area under curve(AUC)value of ROC curve was 0.81.The critical value of the ROC of left C4 stenosis was 1.104,and sensitivity and specificity of that were respectively 70%and 84%,and the AUC was 0.80.Conclusion:The SIR on TOF-MRA can effectively predict the occurrence of extracranial ICAS.
