摘要
肠易激综合征(irritable bowel syndrome,IBS)是消化系统的常见疾病,对患者的日常生活有明显影响,是以反复发作的腹痛、腹胀或腹部不适,并伴排便习惯改变和排便异常的疾病。临床以腹泻型肠易激综合征(irritable bowel syndrome with diarrhea,IBS-D)常见,西医认为其发病机制主要与脑肠轴功能异常、内脏高敏感度、胃肠运动异常、肠道微生物群失衡和心理因素等有关,采取常规药物治疗、心理疗法和饮食疗法等方案。中医认为,脾虚湿盛是病机的关键,并与多个脏腑功能异常有关,本文重点综述IBS-D中医临床常见证型(肝郁脾虚证、脾虚湿盛证、脾肾阳虚证)及经验用方思路和中医外治法经验,提倡中西医结合治疗,并总结中西医治疗各自的优劣,为临床治疗提供参考。
Irritable bowel syndrome(IBS)is a common digestive system disease that has a significant impact on the daily life of patients.It is a disease characterized by recurrent abdominal pain,bloating,or discomfort,accompanied by changes in bowel habits and abnormal bowel movements.Irritable bowel syndrome with diarrhea(IBS-D)is more common in clinical practice,according to Western Medicine,the main causes of IBS-D are abnormal brain-gut axis function,high visceral sensitivity,abnormal gastrointestinal motility,imbalance of gut microbiota,and psychological factors.Treatment approaches include conventional drug therapy,psychotherapy,and dietary therapy.On the other hand,Traditional Chinese Medicine attributes the pathogenesis to spleen deficiency and dampness excess,which are linked to the abnormal functions of multiple organs.This article aims to summarize the common clinical syndromes(liver depression and spleen deficiency,spleen deficiency and dampness excess,spleen and kidney yang deficiency)and the experience of formula and external treatment methods in Traditional Chinese Medicine.It advocates for the integration of Traditional Chinese and Western Medicine in treatment and provides a comprehensive overview of the advantages and disadvantages of both approaches,offering valuable insights for clinical treatment.
